I am thinking there could be a happy-ish option. What elevates this for me from mere wishful thinking to not entirely groundless suspicion is the ME3 ending fallout - I think the initial detonation was loud enough to carry from the ME offices to DA ones. Then again, DA is doing its own thing, so who knows. But as I said, I don't think it's entirely groundless.
Also, didn't a tweet above mention that "all horrible things that happen to you are ones you kind of deserved?" This could allude to the hearing we see the Inquisitor dragged into ("do we really need another keep?" "SHUT UP AND MAKE IT PRETTY") and the impact the Inquisition has had on the world, but the mark - dying for accidentally stumbling into it? Or other, similarly permanent outcome? I don't know, I have my doubts. We'll know for sure in a few days.
Just know that you stand to learn a -lot- more about games by going"I wonder what went wrong?" than by assuming deliberate, malicious sloth.

@Mike_Laidlaw On own stuff, I'd say I tried to make light content engaging and mistakenly made some that felt like it SHOULD'VE had more.

I can imagine certain choices not playing out well and being the cause of fan tears. I can see the fate of the Iron Bull potentially being an area full of angst. If you choose an alliance with the Qunari over the chargers I fully expect Iron Bull to turn on you regardless of whether you’re in a romance with him or not and if you choose the chargers he’s tal-vashoth so the invading qunari probably have secondary orders to take him out or capture him for re-education.

I fully expect that to happen, at this rate. My poor Lavellan was obviously asked to go to the Conclave because of what a terrible apprentice she was.
I also expect Solas to respond to all of Inquisitor's questions with "Spoilers!" Laidlaw and Weekes have tweeted that enough, it's a hint. Hey, they said we could ask him a lot of questions, not that he'd actually answer any of them!
If you took the lore options on the war table you will have an option that knows what he's on about.
