Many plurals of titles/positions do put the plural form on the first word ("Governors-General", "mothers-in-law", 'knights-errant", "passers-by") but only when the later word(s) describe(s) the first.
"Knight-Captain" is a military rank, like brigadier general or lieutenant colonel, and so its plural becomes "Knight-Captains" rather than "Knights-Captain".
(I'm not entirely sure of this though, and I'd be interested if anyone finds a source that puts it as Knights-Captain)

I don't think a "Knight-Captain" is a "Knight" modified by a "Captain" (in fact, I'd say Knight is the descriptive modifier here). If other ranks include sergeants, then a Knight-Captain is a captain who happens, by virtue of that rank, to be a knight also.
The Word of Gaider could sort out this linguistic dilemma!

well in England the plural of Lord-Lieutenant is Lord-Lieutenants. By pluralising the Lieutenant it makes clear it's a military, rather than noble rank (it is actually an honourary postion as the queens representative in the county now, lets just say when Henry VIII first appointed them, they could be more...muscular than simply sorting out who was allergic to what for the meet and greet meal)

OT: to the Knights-Captain X Knight-Captains issue.
The official DA Templar Order ranks are (source: DA wiki):
Knight-Divine
Knight-Vigilant
Knight-Commander
Knight-Captain
Knight-Lieutenant
Knight-Corporal
Templar
English is not my native language, but imho the "Knight" could be substituted by Templar (meaning the Templar Order, all the Knights/Templars), so the "Knight" in the word Knight-Captain means a "Captain (leader) of the Knights/Templars" and not how high/low the rank is X to some common military ranking system, where e.g. a Lieutenant is a low rank a Commander is a high rank and a Lieutenant Commander means it's one rank lower than a Commnader and not that it's the leading person of a bunch of Lieutenants.
So imho singular: Knight-Captain, plural: Knight-Captains

I would tend to agree with this reasoning, that Knight is modifying captain, primarily because I thought they were trying the distinguish Cullen's position from Aveline's. Since he's the Knight-Captain and she's the Guard-Captain, I figured that the prefixes were telling us whether the captain was with the templars or with the guard. :shrug:

Look over there. Behind that rock. Is that??? Oh, Maker it’s poor Ser Cullen again. The lad is cowering and crying. He knows what’s coming bless his heart. What the poor lad doesn’t know is that tears will only whip the Fangirl into further frenzy. They tend to view them as a sign of manly sensitivity you see.
My but Cullen’s been at this long enough. The chap should know better…

Run Ser Cullen!!! Run your knickers off!!!!
(The crew runs huffing and puffing trying to keep up with that action as it plays out before them.)
We round the bend and come upon a tragic and heart wrenching scene. On the ground dear listeners lays a broken and franticly pinned Ser Cullen. We bear witness to the sheer horror and desperation in his eyes as he continues to struggle.
You know maybe he should rethink that armor must make it even harder to get away. Ah but it never helped the Arishok to get away, now has it? Oh well no matter now then is it?
Our predator sits perched on the downed preys back merrily pecking away at a laptop.
Where in the void did she keep that hidden? /sigh another mute point dear listeners…
