A "Hardcore" mode. Permanent injuries if not treated immediately, salves aren't potions and take time for an effect and too much lyrium causes addiction and withdrawal, ect. Stuff like that.
Templar / Mage issue handled correctly, Dragon Age 2 had some elements of the conflict which were good but both sides were horribly portrayed with everybody appearing unreasonable at one point or another and even doing the "tranquil solution" for parallels in reality.
Not straying from the "people are worst than the monsters", Loghain was the greatest part in Origins and ruining Meredith / Orsino at the end practically put the entire conflict which the game had built into nothing but MIND CONTROLLL.
If consequences can't be wildly branched, something along the lines of The Walking Dead (by Telltale) could work where the only thing decisions do is impact how people feel about you and the conversations / scenes which you have with them. Having a character nearly abandon you if you work against him or work to save you can do much in satisfying the player, despite you always getting away in both scenarios.
