My 5 Things:
1. Multiple backgrounds that affect how the game begins and how the world treats my character (if this includes multiple races, that would be icing on the cake).
2. A revamp of the dialogue wheel/paraphrase/conversation icon system that is less restrictive
3. Females for all races, preferably with a few of them being companions (or, as above, a playable character race).
4. Using Mage/Templar war as a jumping off point to a much more (to me) entertaining plot.
5. Less dangling of plot thread lines that are not intended to go anywhere (the Nexus Golem, Golems of Amgarak, Sandal's prophecy, Enigma of Kirkwall, Gaxkang/The Forbidden Ones, the Tome of Koslun, Corypheus, the Architect, etc.) that are essentially thrown in without much thought at all about where they would be going. And I'd also like more resolution to big choices in previous games without reducing them to a number, a small cutscene, or a two second side quest (Urn of Sacred Ashes, Anvil of the Void, Bhelen/Harrowmont, Ferelden's ruler, saving Amaranthine/Vigil's Keep, etc). So... basically I want Bioware to start acting on all of this lore they've been building up. Instead of introducing tons more stuff that won't ever get resolved/answered/properly followed up on, ever.
Pretty much this...

1) Protagonist Origins (human, dwarf, elf, qunari)
2) Factions like the Chantry, Mages, Grey Wardens, that can be supported, ignored or opposed.
3) The ability to own and upgrade a home or headquarters (castle, fortress, mansion, etc)
4) Dialog interrupts from ME2+3, so I can interrupt NPCs with a hug or the murder knife.
5) New Game Plus. I'd love to be able to replay DA2 with Champion Gear set from the start of the game. This goes for all armor, weapons and equipment and for all followers. After I playthrough once, I'd like the option to replay wearing my best looking gear. This is so I can enjoy the cinematic and cosmetic aspects of the game more.

2) Balanced gameplay and dialogue. I didn't really like listening to the characters ramble on and on about one topic in DA:O, and in DA:II there was way too much action, and fighting hoards of enemies got just as boring as listening to Leliana's stories.
3) Immersion. I want to lose myself in the world around me. I want to travel to different cities, towns, and villages. I want to dive deep into dungeons with my companions and forget what daylight looks like. I also would like to see branching questlines, making small decisions that don't seem as important when chosen, impact the world in the long run.
4) Tactical combat. I want to be able to have a rogue climb up to a cliff and snipe the archers behind the enemy lines while a mage hides behind a boulder to heal a greatsword wielding warrior that's distracting the main enemy force. I want the characters in my party to do this automatically, without my input, if at all possible.
5) Recurring characters: Be able to see (or at least hear of) the Warden of DA:O, and the Champion from DA:II. I want to cross paths with the Champion's sibling. It would be awesome if the Warden and Champion could join my party at some point in the game (Maybe in one final epic battle?). I want to know why Flemeth is so interested in the destiny of the main characters! Who is she, really? A powerful witch, or something else altogether? I want to learn more about Leliana's involvement with the Seekers of the Chantry, and if she wants to hunt mages down, or is trying to make some peace between the Chantry and the Circle. I want to know if Zevran had any more encounters with the Crows. I just want to know what happened to all of my friends.
I'm looking forward to Inquisition! (I hope it's a dark and creepy game, as well)

1. please bring back the race choice! But keep all the models like you did in DA2, the quinari and the elves looked AMAZING! I'd loved being able to choose what I was in DA;O and it was fun seeing how people reacted to you differently based on it. Please let us choose to be able to be a quinari too!
2. As many have said, have a much more open world, I liked in DA:O where you could visit tons of cities, in DA2 it sucked that you really were in only one place, and that gets really boring. I would love if we could go to other countries as wel, like Orlais, etc.
3. A more open story line where we can pick and choose what we want to do, but do keep a main quest line that can push us to our next goals. Just so we dont get totally lost and wander around doing nothing but side-quests.
4. Much more options for armor and clothes. In the last games there were barely any options for armors and robes that looked any different. The robes were mostly recolors and you weren't able to have any of the cool tevinter robes with the feathered shoulders. But also, give the mages more diverse looks, some people may not want their badass mage to wear a robe and look like a 60 year old man. Give them some cool mage armor, maybe peeks of scale male covered by loose cloth or something like that. Just give us some options to decide how we want our character to present themselves. Maybe have mages have a way to hide themselves too, so they aren't so easily recognized as mages? When you are just walking around town in robes and have your staff just hanging off your back, its amazing that no one tackles you to the ground to drag you to the chantry.
5. More normal relatable companions. The companions in the last game were all just polar opposites and extremes. The shy girl, the ****, the angsty guy, and the crazy extremist. No one seemed like a beleivable person, not everyone is so black and white. I'd love there to be more realistic people with likes and dislikes and seem like they aren't fan service or meant to just fulfil a trope.
Also on that, please make all of them available to either gender, for a game that is based on choices and caters to your own will, it seems only fair and right for you to be with whoever you want. But, so its more beleivable, make you work for it. Say, if one character seems more attracted to women, then you just have to work harder to gain their trust, affection, and in the end, their love. Please dont restrict characters to either gender pc because for us who like the same sex it isnt fun to be well denied the person you really want for your character thats in a game that focuses well on your character. Romance where they are all available can be done, it just has to be done with more thought.
(I came up with more sorry; )
6. I'd love to see all of our choices from both of the previous games to affect this one, even though that might be too hard to put in the game, it would be really cool to see how our warden and hawke have affected the world and its future. Also it would be amazing to see our Warden and Hawke in this game as cameos or something of the like.
7. Have people notice if you use blood magic. I want my party members to say something, reprimand me or have the templars come after me for it. Put the mage in danger or becoming an abomination, have them have to reject demons, just your mage isn't god. They should have consequences for using blood magic and have it affect those around them.
On the topic of magic, bringing back the arcane warrior and shapeshifting classes would be cool. But having them overhauled and made better, like turning into cooler animals, like even a dragon like Flemeth does or a werewolf.
8. Please do not totally change an old characters looks if you bring them in for a cameo. Like how Merril, Flemeth, and Isabella changed really dramatically. Also, let us be able to kill companions off if they betray us like maybe we could get revenge on Isabella for stealing the book and running off if she did that in your save game, etc. Dont force us to like and use someone that in ME where we had Liara shoved down our throats and weren't able to kill or kick her off if we hated her. PLEASE let our character be able to tell companions how we feel about them. It would be interesting to have a companion you absolutely hated and told them, it would create an interesting drama and plot point.
Please dont rush the game, take another year if you need it. Don't make this like MA3 and have none of our choices matter and have it done shodily. I beleive in you guys and support you all the way!
p.s.= try and let it be for xbox360?
EDIT:
I thought of another about combat. PLEASE leave combat fast paced, it was horrid in DA; O where the mage just spat out a ball of light....and another.....and another...then a crappy looking bigger fireball, in DA2 the fighting looked more exciting, and please let there be NO FRIENDLY FIRE because that is so annoying as hell, keep it like DA2 where the cone of ice didnt freeze half my companions as I used it.

2) more lyrical, more romance, more epic.
3) Power continue to evolve game even if the main story is finished.
4) More sensuality, glamor, seduction in the scenery and characters.
5) activity of the brothel larger. Clearly seen that the hero will be linked to the Chantry, very modest, it would be fun to see what kind of relationship he could keep up with the "professionals" such establishment ^ ^

3. A long main story with choices that affect the ending of the game.
4. Tactical combat. No more of the fast paced cartoony action of DA2
5. A large world map. Multiple hub cities to visit and various other locations. (Like DAO)

2. Romance Options: I loved how DA2 actually handled this, how most companions could be romanced, but there is still more room for improvement. I don't think it should go like ME3 and have 1 for each preference as I believe that would detract from the game in respect that you can't love who you want, but maybe add something like synergy buffs that go beyond the friendship/rivalry scale?
3. Sandbox Ending/New Game+: Nothing frustrates me more than putting in long hours on a game and then once it's over it's over. I personally love how Fallout 3 handled this with the Broken Steel dlc, which circumvents this problem and allows you to keep on exploring and such. I also like how Chrono Trigger handled it. Impressively high level cap, allows you to play over and over and over again, keeping (almost) everything you had from the last save including stats and level and equipment and such. Of course in the latter case some things have to be reset, such as companion friendship/rivalry and romance as an example.
4. Large/Full World: Although there is plenty to explore in DA2, Kirkwall gets a little stale after a while. I would like to see the greater world open up to us, or at the very least a much more significant part than just one citystate. Perhaps at least several areas much like in the Shadow Hearts games, which you pretty much bounce around between several areas in order to accomplish everything, even the main story requires you visit several different places, including places you've already been. That alone I think would open up the world a lot more, just having plenty of other cities/areas to visit and just do things in. Maybe have areas that respawn groups of enemies every time you visit so as to keep the game rolling along after you've played through it. You know, something to give someone incentive to at least casually keep playing in the event of sandbox ending.
5. Choices That Matter: As stated earlier in the Character Customization, I would like my choices to matter. Not just on a personal scale, but on a worldy scale. This was touched upon, but severely limited in DA2. I send Feynriel to the Dalish? Nice. Good for me. Here's some templars in Act 2 I can kill when I visit Sundermount. Help the Starkhaven mages escape by tricking the templars in Act 1? Oops, looks like your choice didn't matter since they were mostly captured anyway. Also a few other quests that have a few other follow up quests in later acts. But that was not quite enough. I'd like it so that maybe there are factions like in Fallout: New Vegas, where if I side with one I lose options for others, but that there are plenty of other factions I can have fun with. This could be most evident in that if you aid someone of one faction, like for example escaping/revolting mages. Obviously your templar reputation will suffer for this, but depending on your choices and actions in the quest, maybe you can increase favor with other factions too. Maybe by helping the problem be resolved relatively peacefully and such you gain renown with the Chantry too for helping to bring peace back to the world or something along those lines.
All in all if they just borrow or adapt some of the good ideas from other games that have alredy been proven to work, this next one will be a shining example of what an RPG should be. You'll still have nit pickers, but those people will always find something to pick at no matter what you do. And sure, I know this in and of itself is sorta nit picking, but it's positive and in the interests of making this next game a better game for all of us.

2. Choices that matter
3. Multiple ending, not all the same
4. Large open world more like Reckoning
5. Don't kill the Hero. That way you could sell more DLCs. If you kill the hero there won't be much interest in the DLC!
I'm big on DLCs. Sometimes they are better than the main story.

2) More NPC's in villages....generally expected tumbleweed and the deliverance banjo tune to play in some area's of kirkwall
3) A sort of addition to a feature such as in awakerning, where if the castle does turn out to be run by the PC then be able to build it up and defend it against say an enermy army, if money or resources is spent = better defenses, a bit like mass effect 3's war assets however they would actually make a visible impact.
4) A banter filled mission, a mission to detract from the inevitable doom and gloom and morbidness
5) AN ENDING! and i mean a real ending....Games developers these days, not just bioware but many others seem to struggle with this recently, set an example of how its done

2. More ability to effect the characters around you. A good example is Alistair/Leliana in DAO and Kaidan/Garrus/Ashley in ME. In Dragon Age Origins, you had chanced to have conversations with each Alistair and Leliana that significantly changed the way they viewed things ("Hardened" them). Similarly, you could influence Kaidan to make him more xenophobic, influence Ashley to do the opposite for her, and Garrus would see Shepard's actions and follow his moral compass to a degree. A big issue for me in Dragon Age 2 was that I never felt like I could do that. I never felt like I could convince Merrill maybe blood magic was a bad thing and have that effect her. I never felt like i could Convince Sebastian that mages deserved better than they were getting from the Chantry. It always felt like I was just choosing whether or not they established a friendship or rivalry with me.
3. Armor for specific characters or outfits. This one is sort of a personal preference, but an example is Morrigan's Robes of Possession. I like each character to have a set (or sets) of armor made just for them that have unique appearances. Seeing my party members in plain chainmail or leather armor made them feel generic to me at times in Dragon Age Origins, and even in a full set of massive armor, the character themselves was lost to the appearance of the armor. I'm not saying to take those armors away, but having armor that is built just for Zevran, for example, which highlights unique aspects of his character design, even if not statistically his best armor would make me enjoy using him and seeing him on screen a bit more. On the flip side, I would avoid going the DA2 route, which saw every character have an interesting and unique design, but took away the player's ability to modify it. Basically, giving each character multiple "outfits" or armors allows the player to exercise some customization over their party, while still allowing each character to feel unique and important.
4. A more diverse party. One of the cool things about a series like Dragon Age or even Mass Effect is that the universe's lore contains so many different races/species. Even if the player character is always human (which I don't mind, myself), that doesn't mean that the party can't reflect the diversity of the universe around them. In Dragon Age Origins, the party had Humans, a dog, a Golem, an Elf, a Dwarf, and a Qunari. In Dragon Age 2, we had Humans, 2 elves, and a dwarf. With the Dragon Age universe being occupied by all sorts of different factions, nations and races, it really benefits the game to make the party reflect that. To get off point a bit, I never felt like the characters in DA2 helped me experience the world more, either. I never felt like talking to Isabela left me understanding the culture she came from as well as talking to Zevran left me understanding the Crows or Antiva. Leliana left you feeling more in touch with Orlais than Fenris did Tevinter. While Isabela and Fenris each had unique origins of their own, you never really got to experience them or understand them to the degree that you did previously. A party with a diverse cast of characters who each make their homeland/faction seem real and grounded would be a huge, huge, huge plus for me.
5. Smarter combat. This is hard for me to exactly figure out, but it would be a balance between the two Dragon Age games for me. I hated combat in DAO because it was so slow and boring looking. I hated the combat in DA2 because while it was faster and more interesting (it maybe went too flashy in that regard), it never really felt like more than button mashing and throwing random skills in there until people died. So I would keep the speed and some of the flash from DA2, while bringing back some of the elements that made DAO's combat more thought provoking and challenging. I don't know how it would translate, but Mass Effect often gave me this feeling. Even playing as a soldier, you'd have to creatively use your powers, switch guns, and combine team mates effectively in order to get through the game's tougher encounters. Speaking of weapons, I think it'd be nice to be more easily allowed to switch between weapons in a class. Rogues for example, have bows and daggers. Right now, most people start the game and have a Bow-rogue or a Dagger-wielding rogue. A slight change to the skill trees or a quick menu like Mass Effect has that lets you quickly change weapons in combat would encourage using both kinds of weapons depending on the situation. The same can be said for warriors, who can use a more offensively based 2-handed weapon, or the more defensive weapon and shield setup. Now I'm aware that you could very well go down both weapon trees and switch weapons in the previous games, but with no way to quickly change weapons and the amount of skill points restricting your ability to make a character that's effective while doing both weapon types heavily discourages it. Allowing characters to switch based on what they need at the time and the resulting diversity in battles would make for more engaging combat, I think.

For me, I find that DA eventually turns into a mosh pit of bodies just swiping at each other, regardless of how well you try to plan, the baddies eventually swarm you anyway. I would like for attacks to do MORE damage to both you and the enemy, meaning that while fights might be shorter, they will be more, um, thrilling maybe?
I remember someone saying on a panal that the combat was going to be more about geography? Choke points and the like? Perhaps the higher attack damage will allow for you to set up an ambush, and mow down the regular baddies, but at the same time have that fear of knowing if your plan goes wrong your screwed. Obviously leveling up and armour would negate the damage the enemies can do, but then you could have larger boss levels enemies that can still kill you.
Say an ogre, for example. Perhaps they're unsteady on their feet, but can do massive damage and have a large health pool? To combat this might beast, you might need to make and place traps, or create certain poisons or use a combination of spells and techniques? I never really delved into the trap or poison aspects of previous games, but perhaps by making it almost a requirement for defeating things, more people would see more of the game, in regards to combat styles.

2. Customisation: This has already been confirmed as far as I know, but armour customisation is something I like to see in RPG's.
3. Story: I'd like to see a deeply engaging story. Alot of the time choices in conversation are black and white. I would like to see alot more conversations hovering around the grey area so you don't know for certain what will happen or the reaction you'll get from who you are speaking to.
4. Characters: Well Bioware always manages to crank out quality characters, however I would like to have the opportunity to learn more about squadmates. Whether through quests specifically about them or just squad interactions. I enjoy the ability to stop and talk to those in your squad and it just helps build a better attachment to the characters. In addition, whoever is the "badguy", i'd love to see someone who effectively outmatches our character in both size and intelligence. Someone who repeatedly makes our character look stupid so that you can get a real hatred towards them *cough* Do and Aeris scene *cough*
5. Romances: I think the romance system of both DA and ME is becoming stale. It is too easy to romance someone, talk to them, say this and there you go. I mean sometimes it is that simple in reality, but the most rewarding at the ones you work for lol. Seriously though, something like during a conversation X says she loves roses. Then later in the game, you come across a store that sells flowers and just buy them roses or whatever. I mean thats still pretty simple, but the next part of this point is what i'd really like to see.
There is never any competition for the affections of the squadmates. I would really like to see squadmates becoming interested in each other if you leave it for too long before making a move. It could provide a whole new dymanic and even have it so that you can lose or even have to fight squadmates. Thats obviously just a basic example, but it'd just be nice to have things not be so simple.

(if you can't do it. steal the code, models and textures from EVE Online devs
2. Dynamic combat. I enjoyed playing mage in DA2. I'm not talking about mechanics but how it looks. I'm tired of playing mage that stands in one place like some old fart. Ofcourse this doesn't mean the enemies should always explode like in DA2.
3. High amount of equipment customisation. And you better make it good looking. Also some character specific armor skins so they can look unique while using standard armor parts. (for PC and companions) The clothes for party members in DA2 were cool but not much customisable.
(if you need example look how the AION armor skins work)
4. No characters from DAO or DA2 (unless they are key NPCs or some cameos) Let it be a new story and new things to discover.
5. Since yelling "Death to multiplayer!" won't really work make it campaign co-op via internet or lan instead of the stupid multiplayer maps where people running around and kill the same enemies over and over again. I would really prefer having human friend in my party instead of some strange not-so-cool-as-you-think animal available via DLC (unless its standard not mutated black cat or a parrot that i can name myself).
Edit:
and one more thing. don't make the ending "three button" based. let it be natural consequence of the story.
if i will be able to reload last save game and see different ending it means you have failed.

1. I think I was 10 or 11 when I first played Baldurs Gate 2, I loved it. Dragon Age 1 feels like a Baldurs Gate succesor, Dragon Age2 more like Mass Effect in space... especially the gameplay.
2. One of the things you tried to change in KOTOR - my guess is - was to not make every choice light and dark, good or evil etc. I understand. But lately in many Rpg's many options are now "grey" which are good sometimes, but if many choices are grey and not black and white then no choice no longer matter since they are equal good/bad. I realize that many problems in the world is in a grey zone, but its fun sometimes to be evil or strive harder to do good choice in game and somewhat being rewarded for it.
