A pretty simple bug here - I've played through Leviathan twice and all of the achievements are still locked.

I have all achievements unlocked except the insanity playthrough, but the dlc achievements aren't showing up on the game or n7 hq. It seems pretty common, so could bioware please fix this

Same here. The Leviathan achivements are "Locked". Rest is fine.

Went through my 3rd playthrough of Leviathan and they are still locked.

Yeah Ive done leviathan twice and still don't have the achievements unlocked even though it pops up ingame saying they are.

Mine just stopped even taunting me with the achievement pop-ups. I hope this does not happen with Omega when it comes out.

Mine popped up, but after I started the game next time they were locked again and I couldn't unlock them again. Even on N7HQ they show up as locked achievements.
