1456: The Inquisitor has a distinct "shout" that stuns/damages enemies or does a variety of traits you desire, but it takes up one of the only three buttons you have to use for combat. Oops!
Edit: Unless you are PC Master Race and can mod the game to having the whole keyboard be whatever you choose.

1464. Broodmothers now have boobs ranging from F-cup to Z-cup sizes. The boobs will be G-, M-, T- and Z-cups.
1465. Whenever all party members fall in combat, instead of getting the 'Game Over' screen, they have to fight the Grim Reaper. If they fail, then the 'Game Over' screen appears.
1465a. The Grim Reaper always needs at least 2 hits to be defeated, because when an attack would defeat it, it is left with 1 HP. When that happens, there's a 50% chance it will instantly defeat the whole party.

1460a. Leliana and others will be judged primarily on their ability to lip sync properly, so that it really does look like they are actually singing. As opposed to moving their mouth strangely to someone else's voice.

1479. Enemy non-boss casters relentlessly spam all of the most powerful PC spells with no cooldowns and no mana expenditure.
1480. Hitting an enemy non-boss melee mob results in 50 points of unavoidable retribution damage
1481. Non-dragon bosses have no AI and do not attack or use specials. Basically they stand around waiting for you to kill them.
1482. If the entire party is dropped in combat the Inquisitor wakes up alone in a random game location.
1483. Basically, the only way to finish the game is to die until you wake up in the room with the final boss, tell the Inquisitor to kill it, go have a snack, come back, watch the end credits.

1473a. One particular dungeon area will involve you jumping over barrels, thrown by a gigantic Ogre. You can either jump over them, or smash them with a magic hammer. You also must jump from platform to platform to reach and slay the Ogre.
1473b. The swimming will be used in another area, where you must swim from one bank of a stream to another, while avoiding being eaten by alligators who first swim from left to right down the river, and then back again. If you get tired, you can rest on logs that float on the surface.
1473c. Gryphons are extinct. So, you will, instead, fly on top of a dragon. That dragon's name, however, is "Puff" and you will have to befriend it and its friend "Godzookie."
Flying will also lead you to discover how many areas of Thedas are unfinished.

1475b. But nobody cares.
1476. All "evil" dialog in the game employs a "no takebacks" design. If you say "eh, go ahead and kill em" that is what happens. If the bad guy says "join me!" and you say "sure" you do not get a confirmation dialog or a chance to betray said bad guy later on. The "good" dialog requires you to confirm your decision upwards of 4 times.

1477a. If the party member hasn't fully regenerated by the next battle, the missing parts will remain missing.
1477b. If a party member loses both hands, they are unable to fight, and just end up in the way and/or being used as meat shields.
1477c. Cannibalism is a great way to return body parts quickly, but they must be the corresponding parts. Other parts won't be cannibalisable by the character.
1478. Mothers and sisters from the nearest chantry will sometimes appear, walking by the current battle. When this happens, they will scold you for not dedicating your lives to the Maker.
1478a. Chanters may also walk by. However, they completely ignore their surroundings, and may just walk right in the middle of a battle, all the while chanting the Chant of Light.
1478b. There is an easter egg battle that will never have such a chantry-goer walk by. It is not the final battle.
