While I am not vehemently opposed to the ME setting exploring new locations outside the Milky Way, I would recommend that any such actions are very limited and have a substantial build up before its reveal.
The Stargate reference is not unwarranted, jumping to a new location, in a new galaxy, in each new episode lessened the impact of traveling across such vast distances "I just jumped 3 quarters across the universe like i was crossing the street.", as well as making each of the new locations seem smaller "Look its that one galaxy with the desert planet! Next episode we are going to that one galaxy with the water world!"
Paragon/Renegade needs to go.
Armors need to be more ME1 like, thermal clips need to go or function differently.

Haven't read all 185 pages before hand so forgive any repeats (but I'm sure the extra data points help).
Exploration
Both exploring new (or radically changed) lands as well as cultures, and not just by being told them, but by being able to play them. If a culture we visit is hugely matriarchal make it more difficult for male characters, if a world has a heavy gravity field, make out characters move slower and tire quicker. Also make sure we arn't just in oddly shaped, decorative corridors which leads me too...
Different ways to the same goal.
Look to games like Deus Ex or Shadow run Returns at how you can break into a enemy held area and eliminate the enemy, you can sneak, skill, speak your way through as well as just going in shooting. This can make more use of the classes, making them more than just different ways to kill the enemy.
Option that make sense
Why where the only options to kill or release the rachi queen, why not just leave her there for the council to decide, why is killing a enemy solider in a interrupt renegade, while killing him 3o seconds later in combat fine? Why is being wholly Paragon or Renegade rewarded when a lot of the decisions are arbitrary or dumb.
Story and scale
Don't try and top the scale of ME3's saving all sentient space faring life otherwise the games will suffer from power creep, the the player character shooting reapers out the sky with their spit, while DA2 failed in it's execution the personal story idea was sound.
Had more but the heat is melting my brain, but ideally I'd like the freedom to explore from ME1, the mission structure and epic final mission from ME2, and the mechanics (combat, inventory and skills) from ME3, and from other games, the freedom to explore from no man's sky, the ability to play your own way from Deus Ex,
Sell me on the multi-player. I am not liking the rumors, I am hearing so far.
I want witches and vampires in space.
-Vast open world littered with activities and distractions.
-Space exploration where the player can pilot ships like the Normandy (not the minigame from previous ME games).
-Seamless integration from space-to-planet and vice versa (No Man's Sky) with atmospheric flight and landing.
-Cooperative multiplayer built around BioWare story and exploring the galaxy (Star Wars: The Old Republic) rather than just copying Firefight from Halo and Horde from Gears of War.
-In-depth crafting providing a large variety of gear to make.
-Destructible and a more interactive environment with more parkour elements in traversal.
-Finishing moves (similar to VATS in Fallout, kill moves in Skyrim, etc.) to give more of a cinematic and epic tone to finishing battles.
-More impactful and believable relationships where my choices (not necessarily dialogue) impact approval.
These are just a few ideas that I believe could really improve and expand upon what is capable of Mass Effect. Lets strive to make Mass Effect the quintessential sci-fi experience we all want it to be.

Why not? Bioware always said Mass Effect is about choices but let's face it: it wasn't as important as we've been told. Time limited choices can change that. I want the game to push the player, I want to feel the urge and the pressure in a dilemma because choices grant you freedom but it also can be a burden. Just like Flamingdropbear said a few post ahead, in ME1 I there should have been an option where you let the concil decide the fate of the Racnis. Adding a timer (7-8 seconds) could easily make the choices more important (or at least they will seem to be more important) The player will obviously ask himself: "what should I choose?" but he will also have to ask "should I choose ?"
I'd rather have the Mako to be entirely optional or 100% customizable. I hated the explorations from Mass Effect 1 for the strict reason everything was so unbalanced when you encountered turrets and enemies. Playing on Hardcore and Insanity mode was a pain in the ass when exploring on higher levels, same as the asteroid from the DLC where there were tons of turrets. Those really made me mad. I tried to explore those places on lower difficulties but the turrets were always unbalanced. Anyways, I'd rather have the Mako optional or 100% customizable like I said, adding the armours, the shields we want and the types of guns and bombs it could use. If it's possible, I'd also rather have an option to scan planets like we did in Mass Effect 3, making everything much faster and not a tedious chore.
I want the next ME to actually feel like a finished product, you know like ME1 and 2 felt like. Where there was so little auto-dialogue and a middle option, you know the sometimes witty one; the one that didn't have Shepard choose a side. I would also like if the tiny things were kept in mind. Example: ME2 attention to the liveliness of character's eyes. You know what I'm talking about BW. You discussed this in one of the ME2 trailers, I remember. We didn't have that in ME3. Most of the time characters looked dead in the face; Oh and what happen to the TEARS!! Guess you thought nobody would notice. But guess what I NOTICED. Also I want the gun I'm actively using to be present inside the cutscene. ME2 had a bit of the same problem but atleast when Shepard was using a class of gun he was trained in he used the one the player had equipped, instead of always using the default.
Here's one last idea, bring back the end boss. Seriously, I saw the concept of the Illusive Man that players would've fought. But instead we get a dialogue battle then chose the color of our ending. I get that this was suppose to be art and you did an amazing job with the colors, I loved the blue especially, but can you revert back to the traditional way of doing things. If you can't, or just won't, could you interpret your art more comprehensively for the layman like myself. Thank You.

How is an open world, atmospheric flight with seamless and dynamic game play, and crafting bad?
For one, with the mako returning, it's almost assured there will be an open world. Secondly, how many times have we seen the Normandy take off, land, dock, and cruise throughout the Mass Effect trilogy? I'm sure many would rather actually fly themselves, rather than watch the same cut scene over and over. Crafting has always been a major element in other BioWare games and would make sense in Mass Effect. I don't see how these potential features are "bad ideas" at all. Why do you actually play Mass Effect?
Open World? MMO? Who knows which way Bioware are going to jump next? Shepard flying around in a reaper? Everything in Mass Effect 2 + 3 being a dream (or nightmare) battle in shepards mind? Maybe Shep him (or her) self being a dream?
MMO might be a good idea. It would not need to be subscription, just do a Guild Wars 2, or release it in parts over a prolonged period. Lets all take back the galaxy shall we? Destiny Style, a little at a time.
Who knows? There are a lot of possibilities.

I didn't say it wasn't a finished product, I said it didn't feel like one. True ME3 technically is a finished product, being that it was published and sold. But just because it was sent to stores doesn't mean it wasn't lacking and felt unfinished and, or unpolished. But that's my opinion anyway. To you ME3 may have been a flawless piece of art like the Devs intended it to be.

Many of the things I want from ME are already being implemented by DA:I:
-I want to be able to switch to playing companions. The fact that aliens have distinct voices and builds may make it impossible to offer multiple PC races so this is why it is extra important that I can play my companions.
- I want a non-binary morality system so decisions can be more complex etc.
-I want to be able to make meaningful choices without having to wait till the third game (i.e. I shouldn't have been forced to destroy the genophage cure in ME1), and for these choices to have diverging exclusive content.
- I want those choices to be tough and not feel like we can just save everyone through completing extra side quests (i.e. Geth and Quarian)
- Female and Male builds on the various alien races. It was not ok that pretty much all the aliens in ME were blokes except the Asari.
Please give the protagonist an ACTUAL first name!!! "Commander" doesn't count as a first name.
