I read on the Mass Effect 3 Wiki that if you scan Reaper-Controlled Areas, there is a risk of being "terminated by the machines". How exactly does this event happen? Am I given a chance to fight back, or does it just happen out of nowhere and I lose?

Whenever you scan a system a "Reaper Alertness" meter will begin to fill up. Once completely filled, Reapers will invade the system and begin hunting you. You must leave the system to escape; if they reach you, you get a mission failed screen and need to reload from the last save (the game autosaves whenever you enter a new system so it's not that far back).
