I just had a game where my slayer started running at extreme speeds, maybe 5 tiems faster than normal, almost hard to control where you're going.

Im not sure how it happend but it might be related to the teleporting under certain circumstances, maybe paired with Biotic charge. I've also noticed that porting on a slayer can make Banshees go mad and start porting at ultra rapid and through walls all over the map.

And last but not least, biotic chage as it works for all Vanguards seems to have the risk of displacing the player from the floor making them go upwards and soemtiems falls below the floor. I know other games with similar abilities have had similar problems.

Maybe a fix would be to reset the players location and reattach them to the ground if they leave it for more than 5 seconds?

The falling through the floor sound very familiar. It just happened to me in a game and after when i was playing a different character one of my teammate had the same issue. We were both playing as a Vangaurd (Slayer and Krogan). Any ideas?

It's a network data loss or corruption issue. E.G., corrupted vertical coordinate and host and guest systems constantly in disagreement. To fix the issues (and not a bandage that doesn't really work) networking software would require a fix. There seems to be a policy against comments on the network issues.
