...like Assault Rifle extend barrel levels 4 and 5 and the AR Omni-Blade level 5 mods.

The Equipment Guide in the wiki has item locations.  I can't tell you where a certain level is because it doesn't work like that.  You pick one up and it increases the level.  Here are the locations and you can see which ones you picked up or missed.

Assault Rifle Extended Barrel

• Priority: Sur'Kesh
  - After the first cutscene with the female krogan, turn around and head
  up the stairs. Between the two doors, on a shelf along the wall, is
  this mod.
• Priority: Tuchanka
• Priority: Thessia
• Cipritine Armory
• Batarian State Arms (2)*

Assault Rifle Omni-Blade

Note: Requires Mass Effect 3: Leviathan

• Citadel: Leviathan II - In the Requisitions Locker, only available after Mahavid
• Namakli: Leviathan
• Despoina: Leviathan - Near the landing area
• Batarian State Arms
• Cipritine Armory (2)

When a store has a (2) next to it, that means it is the second store to get the mod after you buy a level from the store above it.  So you have to buy the AR Extended Barrel from Cipritine before it shows up at Batarian State Arms.

In addition, it isn't noted in the wiki but the AR Extended Barrel at Batarian State Arms, along with several other items, is not unlocked until after you do the Batarian Codes sidequest, which you must resolve with Charm or Intimidate to get the unlocks (and the Batarian Fleet).  Tested that one myself.

This forum has aweful software, I can't get the formatting fixed in this post.  I give up.
