All of the Weapon Mods that I have collected during the missions have vanished from my armory but the weapons carry over just fine. However, these mods are still available in the Citadel Stores and Procurement Interface in the shuttle bay at the start of the NG+.
It's just that the mods that I had before vanished.

This is a known glitch. Hopefully it is fixed in the next patch for Mass Effect 3, whenever that may be. No one knows a way around it, not that I'm aware of, anyway.
