My theory is Flemeth ate them both...

Before getting eaten , my warden and Hawke were up to cool things.

My warden probably chasing after sentient Darspawn , and studying Avernus Research.
I guess sometimes she went to Weisshaupt , wardens over there don' seem to be the funny type , so when she was fed up she got back to Denerim to see Alistair.
I imagine them both ****ing about their duties , and eating a lot of cheese.(Alistair missing her went a bit bulimic on cheese , which is why he looked like that in DA2)

And my hawke traveled with Fenris.Fenris being broody and annoying , Hawke decided he badly needed a hobby .So they went to the border of Tevinter ,to slay mages and free some slaves.
Hawke tried to form a small community of refugee , Fenris was happy for a time .Then Hawke invited Merrill to teach the elves about their history.Fenris became depressed and started talking about divorce (they were not even married , but that sound more dramatic..)
Along the way , Fenris throw a fit ,he hated being near Tevinter (and Merrill) so he and Hawke bought a boat to go all pirate on slavers.
Sadly ,Hawke couldn't stand the ocean (it made her sick) and she also started to talk about divorce...

Anyway , before Alistair became really obese and my warden left him , and Hawke and Fenris finally broke up , Flemeth came along as a dragon and swallow Hawke and the warden (not at the same time obviously)

Alistair and Fenris swore to avenge the love of their lives.But mostly they kept on brooding and eating cheese.

My Mage Warden went through the portal with Morrigan. But if he's back in Ferelden as Alistair says, I'm guessing he's back secretly helping the other Wardens at Vigil's Keep. Probably keeping a close eye on the situation in Kirkwall and pining for Morrigan. 

My Hawkes sailed off into the sunset with Isabela. Even though Varric doesn't say so, I imagine that the Hawke brothers stayed together. Carver continually complains about learning the skills of a sailor, but he's getting better at it as time passes, and may depart for the Grey Wardens again when the time is right. Of course, Hawke gets news of the mage/templar conflict in various ports, but he has a destination or task in mind that keeps him from intervening. Though Isabela keeps her flighty facade in front of the crew things have only deepened between her and Hawke. I imagine that while the seas are rough, Hawke and Isabela wink at each other in place of saying I Love You. 

The crew never knows if the ship is shaking because the seas are rough or because the captain and Hawke are in theor bedroom

Flemeth didn't eat the men because she is lactose intolerant and Fenris is a sourpuss

A mage, Kenrik Amell followed Morrigan into the mirror. Hopefully he is raising his kid, having lots of snu snu with Morrigan and becoming uber powerful.

While Aedan Hawke became a drifter/apostate aimlessly searching for the quiet life. Hopefully he will find a nice girl to settle down with far away from any mages or templars.

No idea what will be the cannon, nor do I really care as that won't ruin anything for me, but my Warden is probably running the show in Weisshaupt since he's a total badass. Hawke probably ran away with Merrill to some small village to protect themselves from the fallout of the Mage-Templar stuff.

My female human Noble Denise Cousland Married Alistair, but after awakenings she was summoned by the Architect, after he had taken saamples of her blood she became connected to him somehow, and now with Anders and Nate she is under ground trying to stop the birth of a powerful darkspwan, Alistair is prolly looking for the Warden since she never came back after Awekening.

Selena Hawke well after she left the final battle (sided with mages) she ran off with Anders, she wanted to help him fight the darkspwan curse inside him.

That is a good question. Where the hell are they? I know it feels like something Flemeth would know or be directly involved since she is the strongest link among them.

I'm guessing they're doing an important mission only a protagonist can do?

So why aren't we seeing Anders and Hawke right in the midst of the Asunder conflict ?

...Because he could be dead?.. Fans would not stant second retcon-ressurect...especially THIS ressurect. Gaider confirmed that dead Anders is...well, dead. So he can't be mentioned in a book directly as someone who exists.

As for headcanon part - I don't know. I'd hate to build something that can be ruined by DA3 in a moment, if they pull any of two out of closet for some Very Important Cameo. I know that they sailed away, but Isabella had nothing to do with it - she ran away with a book.

Zevran invited party to stay at his camp for a while right after mess. Varric arranged a ship(because it's Varric, he can arrange anything). Bethany decided to stay with those little of mages who survived. Aveline could not leave city guards (and Donnik), and (very likely) she had something to do with fact, that guards did not walk patrols towards the camp for those two days they were waiting.

My DA:A Warden is wherever Morrigan is, waiting for his chance to rid the world of both her and the abomination she spawned, should it be necessary.
Hawke is...somewhere. Probably provoking Qunari into chasing him around pillar filled rooms until they die and sending Varric letters to keep in touch.

Hawke ran off with Isabella, even though Hawke had made Merrill move in. Pretty awesome bug. Screw the scrawny elf witch, and run off with the lusty pirate instead!
