I like to see my characters heads, so I am swapping helmets on and off all the time as I need them in combat.  My character gets helmet head a lot -- the helmet is not equipped, but the model stays on the character.

I've actually had it happen when I switch weapon sets too.  My character acts like she is using a bow (the shoot animation plays, the arrow fires, etc), but she is still holding a dagger.

It usually goes away when I reload, and always when I exit to the main menu and reload.

I see comments from people referring to this on other sites, but none here that I could find.  I was just curious if other people also find this annoying.  And of course, if anyone has found solutions that fix the problem in a more reasonable way than constantly reloading the game.

P.S.  Yes I have the latest patch .
