I noticed that the codex contains entries for some major characters in past games, but others (even for characters that you see) are strangely omitted. If a character is dead I guess not having an entry for them would make sense, but for others I encounter, it seems strange to have no entry (i.e. Jack, Grunt, Urdnot Wrex, Legion, etc.).
Is there a problem with my game? Should I be getting these entries somehow? Or were they overlooked. I've received conflicting reports researching the issue online.

have you checked for updates on each of the patches/dlc's ?
and check installed each of them
right click me3 on the origin page select show details, then right click on each of the dlc's you have and select reapir install /check for updates also do this for the main game as well afterwards
if you installed leviathan already but didnt complete the game playthough then theres a decent chance your locked out of getting those info pages (for me i played through 3 x before getting any of the dlc's) but to be totaly honest your not missing out on much, the various char info entries are only 2 paragraphs
