I had the same problem with this achievement too. I also had a problem with the Hopless Romantic achievement which didn't unlock either. Maybe they're just glitched.

I dunno how ancient this thread is, but I just wanted to say I had the same problem with my game but I fixed it. I had cleared my cache to try to get the Blight-Queller achievement to pop (which it still friggin hasn't) and wasn't signed into LIVE while playing.



I updated the game and made sure to sign into the Dragon Age servers, re-played the ending I needed, and the achievement popped. Take that for what it is, I suppose.

How I got it:
Playthrough 1 - Saved by Morrigan, went through the full epilogue, recieved the achievement for being saved by Morrigan, loaded my autosave, recieved the achievement for sacrificing myself for loading the save (even though I didn't)
Playthrough 2 - Loghain sacrificed himself, went through the full epilogue
Playthrough 3 - Alistair sacrificed himself, went through the full epilogue, received the perfectionist achievement.

i'm having the same issue it seems.



i've done this about 4 times today... LMAO



- had loghain sacrifice himself

-then i sacrificed myself but didn't have alistair with me (i think this is key :/ )





am now working on killing of Alistair but i have to bring him with me to choose it because apparently you cannot get the 'ultimate sacrifice' achievement if no one is with you to argue about it. then i have to do it again- right before 'final battle' and tell him i'm gonna do myself in.



hopefully THAT unlocks the US achievement.



and btw- the last game i won.. the credits almost started rolling and the game just totally bugged out and skipped right to the main menu. i thought i did something wrong the first time, but then i won again and it did it again so something is screwy i assume...



ah well, back to Denerim :D

just to mention this as well...



have weirdo problems with weapon swap slots- they always go back to the one i DON'T choose. someone else must have this problem too.



as soon as i exit inventory, POP- the weapons change on characters back right before my eyes.



weird? bug? glitch?

Friendly recommendation: I had this across three different character saves. It didn't pop up when I actually finished it (Sacrificed Alistair), however I reloaded my mage who had sacrificed himself (the autosave at the endgame) and poof! Achievement Unlocked!

I never had to sacrifice Logain at all. I've read somewhere that the patch (the bane of us all) may have buggered this trophy along with Blight-Queller.



On my first playthrough (this was before the patch) I had Alistair on the throne and did the ritual. My second time through (I had the patch on) I had Anora on the throne and sacrificed Alistair. Then I was like 'Oh damn! Forgot to delete the patch!' then deleted it and sacrificed myself with Anora on the throne and Perfectionist popped up. I'm not sure if the patch is a problem for this trophy, but I deleted it just in case.



I think they key was ritual/sacrifice and Alistair/Anora on the throne. If perfectionist isn't popping up for you, try switching who is on the throne and try again, also try without the patch, but if you don't have any endings before the patch you should be ok.
