Non-Return of the Jedi type ending
Non avoidable deaths
Not increaseing character interaction

If you removed game play level ie keep casual as well as normal
Spending resources on Multiplayer that could be used to make the actual game better.Fewer cut scenes
No companion quests
Change of the diolog system - i e the labeling system which tells you what type of choice it is paragon ,romance , renegade ,netural

Having the option to play a character that supports modern Secular Western Democratic Values. It's absurdly out-of-place in the setting and makes about as much sense as playing a character from space. Those things took centuries to develop and would not spring out of thin air in a Medieval Society. Aside from a few somewhat out-of-place lines in Origins, I thought Dragon Age has been pretty good about this so far.
Having a character that succeeds at everything. It removes tension from the story makes the Protagonist far too Mary Sueish to be likeable or relatable. Mass Effect 2 got hit by this problem pretty hard, especially being able to pull off the final mission without casualties. I thought Mass Effect 3 did a great deal to rectify this, though and DA2 was very good about it. It made Hawke more interesting and sympathetic to know that he/she couldn't always save the day. It gave the story a more tragic feel, which is better than the done-to-death "epic" feel that most RPGs go for.

It's something that I think is going to happen, but is going to infuriate me to no end regardless:
Bioware won't be able to deliver anywhere near the correct amount of world-changing differences based on different imports (which won't bother me, because I know they can't and wish the import function would be abolished) and then the BSN fans will wail and moan about their respective Wardens/Hawkes/romances/world states like they didn't see the warning signs with DA2, ME2 and ME3 and like they weren't warned.

If Flemeth is the old lady Sandal referred to in DA2, and wants to use him for some evil plot. Or if she is secretly the Maker or Fen'Harel or someone.
If we don't see Morrigan.
If BioWare kills off a former companion to satisfy the haters, like Tallis or Leliana.
If Tallis/Cassandra is/are not a companion.
Return characters haven't been thoroughly checked to make sure they don't turn out like Alistair's nose or Zevran's eyes.
The elves have been badly redesigned. The noses are flat while the rest of the face is curvey.
Female armour is not sexy.
People make mods that are clear rip-offs of in-game armour, by removing pieces of the female armour, like they were strutting around their house with less apparel, in the summer.
This would probably annoy me more than most things: ugly PC specific armour like this:

I couldn't find the male version, but if you haven't seen it, trust me: it's revolting! You call that a mage robe BioWare???
Also, players who show off their ugly PC, like they are some sort of sex god/goddess. Seriously, some people just have no taste in character design, and they should not be allowed to make me want to vomit in my mouth.

I agree somewhat on the first, if you're going to introduce a character who thinks like that, or if you let the PC hold those values then they need to be remarked upon as odd and be given some sort of explanation. I'm sure there are countries and city-states and such we haven't met that may hold those sort of values, and it would be interesting to see characters with wildly different ideas about how society should work working together.
I disagree on the example of the Sucide Mission, since it felt more like it was Shepard actually forming a proper team and knowing how to use it than anything - but I really don't want a character than can persuade everyone to their side with a single word and be "awesome" from the get-go.
