Silver Mission - Full extraction of the  same players through out  the mission -all 4 players with scores 6+ or better. 

Then I was not listed in the final mission credits as having even been on the mission, but there was a new name of a player who was never in the mission or showed in the running tally on the screen during the mission.   Is this an odd glitch or does this happen regularly?

its a lag based glitch that sometimes occures when someone dropped from the lobby just at the point of launch and occasionally you cna have a game wiht 5 people in it due to the bug, only 4 will be listed on the end screen and they might not necessarily have been in the game at all

it was listed as one of hte fixes wiht the patch though and since the patch i havent seen it ( used ot happen all the time when playing wiht a host from russia but last night we had a faultless condition 9 times from that)
