So, I've actually seen a few problems with the endgame so far.  For instance - Loghain refuses to die.  Whether I or any party member kills him, after the coronation of either Alistair or Loghain's daughter - he shows up in the crowd waiting to speak with you.
Then he makes a really weird appearance - when choosing your team for the final battle (Loghain's dead - so there's no option to choose him) - he comes strolling through the line to offer his well wishes, except he's invisible!

I've seen the invisible companion thing a few times actually - once was on a play thru of Orzamarr - where Oghren couldnt' be found anywhere - so I proceeded to do the quest without him.  At one point it stopped and played a cutscene where Oghren should've been there.  Sometimes when they're invisible you won't get the voiceover, or even the text - there's just a pause - one long enough to allow them to say what they're saying.

Speaking of Oghren - I can't get him to show up - no matter how hard I try or where, and I've been sure to follow the other peoples suggestions for getting him to show up.  Nada.  The only way I was able to get him into the party this last playthru was to add him in via console.  I hate doing that - but I was curious.

Aside from being irritating, it doesn't really affect gameplay too much.
