Was just on my Asari Vanguard and I was killing stuff all over then on about wave 5 I could not damage anything nor use any skills/consumables. I finally left the game on wave 8 after spending 2 or so rounds trying to get killed hoping that would fix me. While my ammo lasted I was able to damage enemies a little and I never died the entire game before I left so it was not the vanguard bug AFAIK.

Known issue. I believe some network flaws cause this, putting it into an invalid state. The vanguard fixes probably addressed the way events come in and are processed, rather than the network itself. It is conceivable that the same sort of fix could be made for each glitch, or that they could also improve state reset upon death to be more comprehensive...as a workaround. It's unlikely they can do anything without a full patch though.
