In DA system we got injuries when we "dead" then "alive" again. before we dead we got wounded so badly without any "injuries". How about this time injuries applied to percentage of health we have, i mean when health percentage drop we got an injury and penalties

100% health - No injury
80% health - open wound -1 health point per seconds
60% health - broken rib -5 strength points
40% health - broken bones -5 dexterity points
20% health - broken skull -20% movement and attack speed
5% health - coma/fainted/ko

KO at 5% so the character is not considered as dead, similar rule apply to enemies. So when everybody KO game over, if we fight enemies only 1 enemy not KO and we spare him/her/it, all the rest get up and walk away

That lead to healing magic

level 1 healing - heal X% health only and not injury
level 2 healing - heal X% health and one injury
level 3 healing - heal X% health and two injuries
level 4 healing - heal X% health and three injuries
level 5 healing - heal X% health and all injuries

And potions also deal the similar thing like magical healing but to lesser effect than magic.

So that would be challenging and realistic isn't it?
