I first noticed this last night when I was playing MP, and suddenly got a little pop up saying 'Mail Shot 1/10' and 2/10. At first I thought this meant for the new MP Achievement system....apparently I was wrong.

Checking today, I found out that ALL of my achievements were wiped, which is very disconcerning given the fact that they carry over to the new MP achievement.

Prior to yesterday I had unlocked every single Achievement in the game, but now I have suddenly lost all of them. Is there any way to fix this?? And is this a problem on my hardware/software, or is this a problem in BW's servers?

I had the exact same thing, but for me it happened a few months ago, when, for an achievement, you needed to promote several characters. After I did that, my achievements were all gone. I thought it might have been temporary, but with the new N7 HQ (http://social.bioware.com/n7hq/),
I just now saw that they're still gone. I still have some old saves, maybe restarting from one of those will clear it up. I had ALL achievements :'(

Had a similar experience, but it only affected my Leviathan related achievements (they now appear as locked on the N7 HQ website).

If you may recall there were two closely spaced origin updates a while back...not sure when, 1 to 2 months ago. The first update forcibly turned cloud back on if it was off, and if you clicked through the update and let it pick defaults after that, then cloud would have downloaded and overwritten local files...having not had cloud on before, this means it erased your progress. Shortly after that another update came out and seems to have stopped the forcible cloud on. All you can do is play again for the achievements and be careful during any options of origin update that you do not choose to overwrite local files.
