I've been looking around at merchants in act 3 and nothing really jumps out at me for my vanguard two-handed warrior.  Any ideas?

He's currently using Pendant of the Morning Frost and the High Lord's Belt, but he's been wearing them since the beginning and I assumed that I'd trip over something by now.

I'm not sure what you were specifically were looking for but if you wanted to stay with the same setup (high physical damage and some sort of resistance) for the amulet, I don't think there's one that goes any higher than the 7% physical damage you have now. I have the DA II strategy guide and none of the amulets listed go any higher than 4%. If you want to keep high attack/high defense for your belt, there's an item called "Ferelden Circle Sash" (+1 willpower, 38 attack,19 defense) that's supposedly available by looting Evelina's corpse in the "On The Loose" quest. However, according to the Dragon Age wiki, as of patch 1.04 it doesn't drop. Go figure.

So I guess for now it appears as if you have the best amulet and belt currently available if you wanted to stay with those effects. Hope this helped!
