It's an odd bug that was created in a patch. Basically, whenever enemies spawned and/or noticed you pre-patch, they'd run at you at once. I was convinced it was a bug. It didn't happen a lot but it happened enough to confuse me. The enemy was weak (and not resistant to anything) and getting hit with every kind of move, but it wouldn't go down. It was some kind of weird lag.
BioWare's fix to that problem was to make enemies wait for a bit before they activated. They'll just stand there for a little while at first. However, since they haven't been activated yet, that also means they don't have HP yet. If you hurt them, their life bar will update as if they've taken damage while in reality all the damage you just dealt disappeared into thin air.
Basically, all enemies post-patch are invulnerable until they start moving, and the life bar lies.
