No matter what I do I cannot finish the Fade. I havegotten Wynne out, but I cannot complete the other two. The Templar's Nightmare is the only one not completed with the Golem. I cannot get to Alister or Leliana. I am in the Fade Portal room and I no not where to gtot next. I will be damed to have to start over again. Can some please, please help me. Thanks. I can't even figure it out with a damn walkthrough. I am so fed up. Please help me, before I go mad. Thanks.

Templar's nightmare u have to change between mouse form, & spirit form form to get to the Islands and different modules. It's a little tricky

Yes, I have all four shapeshifters. Here's the deal, one last time: The Templar's Nightmare is still not finished, however, the others are. I cannot get to Alister or Leliana or the center island. Everything else is completed.

The Templar's Nightmare: The first room
contains a book with a codex, a mouse hole, a fade portal, and a door. Use the
door, and go into the first room on the left to find two Arcane Horrors, some
books, and a mouse hole leading to a Blight Wolf (which gives the Ghoul codex)
and an Essence of Dexterity. The next room down contains another Arcane Horror.
Dispatch of it, and use the mouse hole to the south.
An Ogre and some Abominations will greet you in this room, after you finish them
off, step out into the hall. The middle room will require Burning Man to enter,
it contains a mouse hole, which leads to the next room. Take the mouse hole or
smash the massive door to enter this room and find two Rage Abominations and a
Spirit Door. The Spirit Door leads to the third part of this level, and more
abominations. The door leads out to another room, containing enemies, and a
Fade Portal. Use the portal to be transported to another part of the level, and
switch to Burning Man before the flames overtake you. Through the next door are
two Desire Abominations, and the next door contains an Arcane Horror along with
more Abominations.
In the next room you meet this level's demon, who wants to play hide and go
seek. Turn into a mouse and follow her. Spirit Form will help resist her
spells. An Essence of Cunning waits for you after the battle, and the Fade
Pedestal is your way out.

If I'm reading this right you have an incomplete non-party member dream realm - Templar's Nightmare. You need to finish all the non-dream levels to reach 2 of the 3 party members.

I did find the fade the hardest of the quest to do, and I still find it hard after doing it a few times. The first time I was ready to scream. I started using the map more and figuring out where each mouse hole and spirit door went. Don't give up and good luck.

I see someone already gave a description, but sometimes different wording can help, or people use different methods and routes so:
Where you zone in there is a book to examine for a codex entry (unless you already got it as a mage). Then change to mouse form and go through the northeast mousehole. Change into spirit form and use crushing prison on the arcane horror there.
Change back to normal and walk through the regular door here to the hall, enter the next room to the north (on your right), and kill the sloth abominations. Get the codex note here. Change to mouse again and enter the mousehole to reach another room, change back and kill the blight wolf, and claim the essence of dexterity. Go back through the mousehole, return to normal, step into the hallway, and return to the south room.
Transform into mouse again, enter the mousehole in the southeast corner. When you emerge, use your favourite form on the ogre (some hunger abominations may join in, coming from room next door). After they're dead, switch to burning man and go out into the hallway, enter the next room to the north (on your left).
When past the flames, change to mouse and enter the mousehole against the far wall. Change and kill the rage abominations. Change to spirit form and enter the spirit door.
Kill the enemies here and enter the room to the west. Break down the massive door or use the mousehole to reach the next room. Kill the arcane horror and shambling corpses. Shift to burning man and use the Fade Portal.
Draw the abominations in the next room into this room to burn themselves to death (you can use a fireball, or simply run into their aggro and just run back into the flames, they will follow, hehe).
Continue west, killing enemies, then use the mousehole in the last room to reach the lair of Vereveel. Use spirit form or burning man to destroy her. Once she's dead, activate the essence of cunning. Then use the Fade Pedestal to travel to your companions' nightmares and free them.
Then go to Inner Sanctum.
Urgh - long, but I hope it helps.
Edit: a lot of people seem to have problems with the Fade, not that I can blame them. It took me several times through to find everything (if I did, I hope I did). Maybe I should make a post with my route through the Fade, lol?

Tis is a real fun caption, with the shapeshifting and all. IM just there again.
So i can describe. You dont need a golem to finish this, so you cant be stuck there. It is 5 "sections" you must cross ,easy to follow on the map that way...
First get in the mouse hole on Southeast corner.
Next section, enter the ghost door in the Northern corner. Need fireform to pass a doorway there.
Next section, enter the fade portal in the West corner.
Next seciton, enter the mouse hole in the West corner (following the demon!) Need fireform to pass a doorway again.
Last section is the goal.
There, it couldnt be easier.. pfft lol
