It seems to me that before the characters hit the 'choosing' point, all affairs are openly conducted (the companions know about them and talk about them). There's no 'cheating'. Once two or more hit that choosing point, there's no option to continue. Zevran seems to be speaking as if there was an option there (although one he doesn't let you take) to have a secret affair wtih him. I suppose that's honourable. You are all living and working together, and a secret affair would be pretty tacky and unpleasant. He does consider the other person's feelings.
(I think the implication is there, that if the other person didn't mind, he'd happily carry on sharing)
He's also not sure of his own feelings at that point, so he doesn't ask the warden to choose him (unlike the others, I think. I'm trying to remember their conversations. I'm pretty sure Morrigan says that she won't share. I think Zevran is the only one who doesn't bring his own feelings into it). As usual, he leaves it up to the warden, and doesn't try to make a case for himself.
That's the way I see it, and I suppose the Isabella case was different because he wasn't living/working with her husband and didn't need to consider his feelings. You get the idea that this is fairly new to him. I think it says a lot for him that he does respect the feelings of other party member when it comes to love/strong romantic feelings.
ETA: I agree that it's not obvious what he means by being a 'cheat'. It puzzled me. I've thought about it a lot and I'm still not sure I've got it right.
ETA2: And if it's a case of what triggers the conversation, then yes, it wouldn't be an issue if he didn't care for Isabella and/or her husband didn't care for her. What Isabella felt woud be irrelevant, as that seems to be the case with the warden. If the warden is in love with the other person, or doesn't give a jot about the other person, it makes no difference. It's the other person caring (while Zevran does) that forces a choice. I'm confusing myself now.

I agree with you... he cares, but he doesn't like to talk about his feelings and he doesn't want to burden the Warden. I agree more after digging around in the toolset. I'll post most of the Ali/Zev dialogue here, and I'm sure I'll have to edit this post because of the stupid formatting

Zevran: Isn't it? I've watched you and he together. I know a complication when it rears its head and threatens to bite. ("who are you kidding?")

3. PC: We've "had our fun?"
Zevran: Have we not? I enjoy our time together, but it is not so simple as that. ("let's be realistic")
4. PC: If that's what you want.
Zevran: What I want? If only it were so simple. (glum)

Zevran: I make no claims upon you, nor would I dream of such. You are free to pursue your fancies as you desire, and I would have it no other way. (being very reasonable)
