I think it is both, in that there is friendship (and benefits) and commitment. It's the love part that isn't acknowledged by either of you.
The 'married in Antiva' comment, I can't imagine a PC making seriously. Why would they think that it would mean that? How could an earring exchange in Ferelden mean a couple were married in Antiva? (Maybe it's possible, but I don't see why the warden would think that). So it only works for me as a jokey comment. (I seem to remember a similar option when Alistair gives you the rose). It might be a romance option, but it's a deflecting one (the PC is deflecting). (I suppose if the PC really believed that they were married, his answer sounds bad though. But I don't think he would guess that the PC believed that).
I love the way he looks when he says 'I have no better way to say it'. I suppose you could take that as an admission of his inablity to talk about love. But he looks so happy that you've accepted his gift.
