I just spent about 20 minutes modifying Wynne's tactics. I made sure to save each time I switched from one preset to another. Then, I went back in to look them over after completely exiting the "tactics" screen and all my changes were gone.
I tried multiple times adding simple tactics like adding "enemy with health less than 50%, attack" to the last slot in the "damager" preset. I saved the tactics, exited the screen and when I came back in, it was gone.
What's going on?

You sure saving into a custom set? I dont know i dont use it really, but thats my idea.

Problem is you may not understand the tactics at all. You're only allowed 1 custom set for the game and that's it. Everything else is predetermined on what you have and switching to them is not gonna keep your custom tactics active.

Hm? I see you have predefined sets (lot of emptyness but i dont care), and 3 custom sets you can save to, Custom1, Custom2, Custom3....

You're not hitting O twice after you set them, are you? 
O brings up the pop up asking if you wanna save... X saves and O cancels saving. I cancel saving on accident sometimes when I'm not paying attention.

Hm weird, maybe we have different Version. Only the preset will change in amount, sometimes 2 some 5. I thought you can rename the Custom ones even, havent tried.

Your companion can only use 1 tactic preset during battle. Modifying a preset in the game will automatically turn it into a 'custom tactics set'. You only have 1 custom set for each companion. So just choose one preset to use, but remember that if you modify that one, it will automatically turn into the custom set, and that is the one that will be saved.
