I've had Zevran betray me in Denerim, but no one else (Alistair aside) has ever betrayed or even left my party.
Who else will do this?

These are the ones I'm aware of
If Sten's approval is below certain level, he'll turn hostile once you enter Haven. Defeating him in the ensuing duel will make him apologize and rejoin the party.
Wynne will become hostile if you decide to kill all the mages after talking to Cullen in the Broken Circle questline.
Wynne and an unhardened Leliana will become hostile if they are in the party while defiling Andraste's ashes. Wynne will leave if she's not in the party the next time you set up camp.
Shale will turn hostile if you decide to side with Branka.

If you tick Shale, Oghren, or Zevran off enough, they attack you in the camp. (Says YouTube, anyway. I've never ticked them off to that point.)

There's also a circumstance where Alistair will leave the party for good and tells you off really hard just beforehand.
I don't know, though. With Alistair, if it's an Alistair hate game, it actually is worth just getting the hate speech while keeping him in the party, though you can actually get both. I just like a particular dialog choice that comes up in the hate speech if you hold off until late game and make a certain decision regarding his fate.
