This guide does not pretend to be the only, or even the best, let alone the fastest way to do the Fade segment of the Broken Circle quest. However, it is as complete as I could make it. A friend persuaded me that posting it might help others as 'lost and confused' as he was. So, here goes.

Where I have listed a form, it is generally the form needed to perform an action (like break down a door) or survive something (such as flames). In fights, the forms suggested are just that, suggestions based on what I find effective. What will work for you will depend on your character, their level, their class and build, their equipment, your preferred style, and so on. Save frequently to allow reloading if things go awry.

This is not meant to be a guide for those who can solo nightmare, but a general guide for those who find puzzles confusing, or who want to gain all the permanent attribute increases but can't find them all. That said, I hope I haven't missed any. I have numbered the attributes arbitrarily, based only on the order in which I can 'activate' them. Mages may have already found some or all of the codex entries mentioned.

Lost in Dreams

Weisshaupt

* There is an Essence of Willpower ahead, but you cannot access it until you gain Spirit Form. Go straight ahead, confront Duncan, and kill the false wardens.Use the Fade Pedestal to travel to the Raw Fade.

The Raw Fade

* Talk to Niall.

* Use the Fade Portal (not the Fade Pedestal) to travel to the next island, and kill the rage demon there. Talk to the Mouse and gain Mouse Form. Enter the nearby mouse hole.

* When you exit, change form (back to normal), kill the demon here, examine the Strange Statue, and take the Fade Portal.

* Kill the three demons here, continue to the end, curving up the path, and use the Portal to return to the first island with Niall. Talk to him about being a mouse.

* The Fade Pedestal is now active, allowing travel to the Darkspawn Invasion.

The Darkspawn Invasion

* Walk through the northern door, then go east, killing all the darkspawn until you reach the fire barrier. Then go back to the first room.

* Take Mouse Form and use the mouse hole in the northeast corner, leading to a small t-shaped room. Go straight ahead to another mouse hole and enter it to find and Essence of Cunning (1).

* Go back through the mouse hole and turn right at the intersection, using the second mouse hole in this room. Change form, and kill the two darkspawn here, then travel north through two doors, kill the hurlocks, and use the
mouse hole on the western wall.

* Change form and kill the genlocks in this area, then use the other mouse hole. Kill the darkspawn attacking the Templar Spirit, and he will then grant you Spirit Form. Change to Spirit Form and use it to collect the Essence of Willpower here (2).

* Use the Fade Pedestal to travel back to Weisshaupt and gain the Essence of Willpower there (3) that you passed up earlier. Use the Pedestal here to go to the Burning Tower.

The Burning Tower

* Change into Mouse Form and use the mouse hole on the west wall to claim an Essence of Cunning (4). Return to the previous room and change form again.

* Walk through the door to the northwest in Spirit Form, defeat the templars using Spirit Form or change back to normal to do so. Head up the stairs and through the door.  Turn right and kill the burning hounds, then go left, then right, killing enemies as you go. Change to Mouse Form and use the mouse hole ahead.

* Defeat the burning demon and the templar dreamer, who will then give you Burning Man form. Don’t use the Pedestal yet. Instead, change to Burning Man Form and use the door to the south, turn west, walking through the flames. You will see a Spirit Door ahead. Change to Spirit Form to use the door, thus reaching a room with and Essence of Cunning (5) and an Essence of Constitution (6).

* Reverse your steps (using Spirit Form for the door and Burning Man for the flames), going back to the room with the Fade Pedestal. Use it to return to the Raw Fade.

The Raw Fade Revisited

* Use Spirit Form to enter the Spirit Door here, and defeat Yevena and the two shades. Spirit Form’s skills are good here – Crushing Prison on Yevena first, then Winter’s Grasp on a shade, then back to normal to finish them off. Change to Spirit Form again to claim the Essence of Willpower (7). Examine the Twisted Structure for a codex entry.

* Go back through the Spirit Door and travel east to the Fade Portal. Use it. Change to Mouse Form and use the mouse hole. Change back to normal and step through the Fade Portal, then change to Spirit Form and use the Spirit Door. Kill the wisp wraiths and claim the Essence of Dexterity (8).

* Go back through the Spirit Door, and take the Fade Portal to the east to go back to the first island (again). Use the Fade Pedestal here to travel to the Mages Asunder.

The Mages Asunder

* Change to Mouse Form to enter the mouse hole to the left, and examine the Font of Strength (9), and then the bookcase for a codex entry. Return to the first room. Enter the other mouse hole and sneak up to the two mages. Change back to normal and kill them.

* Go out the door, turn south, and enter the next room on the left. Kill the mages here and change to Spirit Form to claim the Essence of Magic (10). Change back to normal and examine the bookcase for another codex entry. Return to the hallway, and go to the next room on the left, killing the crazy mages.

* Change to Burning Man and exit to the hallway again and go through the door ahead, killing the three servants here. Go through to the next room, where rage demons will appear. Once past the flames, change to Spirit Form if desired and kill them using that form’s skills. Change to Burning Man again to go through the northwest door and
through the fire barrier to the next room.

* Kill the many mages here. A helpful tactic is to enter room far enough to trigger the mages' AI, then run back into the burning room and along wall a little. The mages (or some of them) will follow you into the burning room
in order to attack you thus damaging or killing themselves and making it easier to thin their ranks. This can be done several times if necessary.

* Now move northeast to the final room of mages, killing them. Examine the book and the bookcase for codex entries, then head up the stairs and through the door. In this room, some chantry priests and stone golems are attacking a Cursed Dreamer. Kill them all using normal skills, or use Burning Man’s Fireball and/or Spirit Form’s Crushing Prison and Winter’s Grasp. Afterwards, the cursed dreamer will grant you Golem Form.

* Change to Golem Form and go up the southern stairs; kill the mages and stone golem here. Examine the note and the pile of books for codex entries. Then use Golem Form to bash open the massive door to the south. Kill the priest and mage. Head south and enter the first room on the right. Kill the priest and stone golems, and claim the Essence of Willpower (11). Examine the pile of books for a codex note.

* (As you travel through the halls, there will be several golems to defeat along the way.) Return to the hallway and go northeast to fight some mages and priests, and claim a codex note. In the room to the south of that, kill the stone golems and examine the Font of Strength (12) and another codex note.

* Go south again and use Golem Form to break down the massive door. Kill the two arcane horrors, and then change into Spirit Form to use the Spirit Door. Use preferred form to kill the stone golem here, and then claim the
Essence of Dexterity (13).

* Change to Mouse Form and use the mouse hole to travel to the next room, change form and kill the stone golems, and claim the Essence of Cunning (14).  Use Golem Form to break down the massive door and gain access to the Essence of Dexterity (15) and the Essence of Constitution (16) in the next room.

* Reverse your steps to the room where you fought the arcane horrors. Go south to the final room of Mages Asunder to fight Slavren. Golem form can be used to defeat him. Use the Fade Pedestal to go back to the Darkspawn Invasion.

The Darkspawn Invasion (Again)

* Use Golem Form to break down the massive door to the north, and enter a room with darkspawn and kill them all. Head to the northernmost room, where you will find Uthkiel the Crusher. Kill him using preferred form.

* When he’s dead, do not use the Fade Pedestal. Instead, travel all the way to the southern end of the Darkspawn Invasion, using Burning Man to avoid death in the flames. Just north of the southernmost room is a massive door
– bash it down using Golem Form and claim the Font of Strength (17), then go to the southernmost room and use the Fade Pedestal to travel to the Burning Tower.

The Burning Tower Once More

* There are two massive doors here. The first is to the north, use Golem Form to bash through it and change immediately to Burning Man for the flames. Kill the shambling corpses and burning templars.

* In the room northwest of that is Rhagos. Use Burning Man to fight him so that you don’t take fire damage, but use normal attacks (he is immune to fire attacks). Crushing Prison is also good. Once he’s dead, examine the Font of Strength (18).

* The second massive door is far to the south, down the stairs and back to the circular part of the tower. Use Burning Man again to travel safely through the fire. At the bottom of the stairs there are three doors, the one on
the right is the locked massive door, use Golem Form to bash it down and claim the Essence of Magic (19).

* The Fade Pedestal you want now is a little further south; use it to travel to Templar’s Nightmare.

Templar’s Nightmare

* Examine the book for a codex entry, then change to Mouse Form and go through the northeast mouse hole. Change into Spirit Form (if needed) and use Crushing Prison on the arcane horror here. Change back to normal, go through the regular door here to the hall, and enter the next room to the north (on your right) and kill the sloth abominations. Get the codex entry.

* Change to Mouse Form and enter the mouse hole to reach another room, revert to normal and kill the blight
wolf. Claim the Essence of Dexterity (20). Go back through the mouse hole, change to normal, step into the hallway, and return to the first room.Change to Mouse Form again, and enter the mouse hole in the southeast corner.

* When you emerge, use your favourite form on the ogre (some hunger abominations from next door may join in). After they’re dead, change to Burning Man and go out into the hallway, and enter the next room to the north (on your left).

* Once past the flames, change to Mouse Form again and enter the mouse hole against the wall. Change to
desired form and kill the rage abominations. Then change to Spirit Form and use the Spirit Door. Kill the enemies here and enter the room to the west. Use Golem Form to break down the massive door – or Mouse Form to use the mouse hole and reach the next room. Kill the arcane horror and shambling corpses.

* Change to Burning Man and use the Fade Portal. Aggro the abominations in the next room into the room full of flames you are now in and burn them alive. Continue to the west, killing enemies. Use the mouse hole in the last room to follow Vereveel. Use Spirit Form or Burning Man to annihilate her. Once she’s dead, examine the Essence of Cunning (21).

* Then use the Fade Pedestal to travel to your companions’ nightmares to free them.

A Nightmare (x 3)

* Free your companions.

The Inner Sanctum

* Finally, travel to the Inner Sanctum and fight and defeat Sloth with the aid of your companions. Defeat all of his
forms, and return to the tower.

Edit: I hope I've fixed the formatting. And the colour was a good idea, thx. Why oh why isn't there a 'preview' button?

Looks good!
A suggestion:
Maybe you could use a different color for all the attribute bonuses. For example:

There, you'll find Duncan's Beard of Strength.

Just a note on the the 2 times I've played through Lelianna's Nightmare on the PS3 it's slightly bugged in that I'll answer the Questions to break her out of it and fight the Demon that's tormenting her. After the defeat I'll get the same conversation cutscene again after the fight, then she realizes it's wrong (Cause no fight triggers this time since the demon is dead) and "Wakes" up.

That happens on pc too, and not just in the Fade. I've had it happen twice in the werewolf lair, fighting Zathrian, and several other places as well.

Great Guide, that was a frustrating adventure, When I encountered the Sloth I was all alone, did some research and found that i did not free the companions correctly. Had final Demon down to a couple of strikes solo but got caught and died, boy it is was frustrating. Finally passed with the help of my friends.

If you leave your companions in the Fade, do you lose them permanently?

No. You don't leave them. If you don't free them and you defeat Sloth alone, you all wake up anyway.

great guide, and for those thaqt really dislike the fade ,like me, theres always the Skip the fade mod. which allows you to only free your companions ,do the sloth and leave, giving you all the extra skills,xp and codexs you would get if you did the entire fade
