The idea seems rather silly as you can select your character's sex at the start of game typically. The D&D item was generally used to shake up sexist players (much more commonplace in times past) or as a gag.

If done in the same manner as the mirror in DA2 I agree, but if done in the manner of a cursed item as was done in previous games it could be accounted for with much less immersion breaking.Allan Schumacher wrote...

Baking is logical. If Sten can no longer fight, and we assume that Sten isn't the type to sit around and do nothing, that leaves the passion for cookies, leading into baked goods in general.

Changing hair or make up is one thing; people don't react to it.Changing sex (or gender as the OP says) would be a bigger issue. NPCs and dialogue can change based the player's sex, and that just leads to way more potential issues to crop up.
If you want to change, then just start a new game. The only reason to have it in the game is so that the player can troll their party members, and that just seems like a useless feature.

I honestly don't care whether or not a feature like this is implemented. It's not derogatory, but instead conveniently nonsensical and harmless.

Why do you want to change your gender during a playthrough. Why not start a new playthrough?
The player could change his PC's gender, and then retcon everything that happened before.
This particular Pandora's box was opened as soon as people started expecting respec options. Changing the character's sex is no different.

He at least accidently brought up something not laughibly silly, the ability to chanbge apearance returning, aka the magic mirror, THIS needs to return in EVERY BIOWRE GAME MADE
