 This might be just a one-off, but I just entered a game on Jade (from the start) and everyone was shooting at nothing. Then I realized that I couldn't see the enemies (Collectors in this case).
Also on the 'Squad Elite' challenges I have completed 'Bronze Extractions' and 'Survival Medals' but it's only showing one of them as complete (1/8).
One more thing: I have all the Achievements for both campaign DLCs but they're showing as locked on N7 HQ.

It happened to me just a few minutes ago on Condor. The rest of my squad was firing into nothingness. I was getting hit and my drone (engineer) was just floating and taking damage. One member exited and then my screen froze and all I could do was turn off the console. It's been a few months since my last lockup. I've been tagged as a quitter because of ME3.

i was on the firebase rio and the same happend to me and there wasnt the kills on the bottom of the screen either. something else thats rattled my cage is when i am the asari justicar i cant shoot, use my powers etc,the only thing i can do is move around but when i heavy melee it puts it back to normal,and it has happend with other characters aswell.
