FF14 seems like an interesting case. It was a game that wasnt released when it was ready, and the devs actually decided to redo the whole thing, free of charge to older players.
Thats actually pretty awesome, though i never played any FF games, its still hunble of them to admit their mistakes.
RE6 seems to be because of player expectations, but some love it, and i dont think its anywhere as bad as ME3
FF was a Japan thing. If they screw something up they actually want to fix it where as the West want to limit the damage.
The company I work for is similar.

For some strange reason, people just don't like Raiden.
The problem wasn't Raiden, it was the fact that it was a major bait and switch. They advertized it as the return of Solid Snake, then early in the game you play as a new character. People expected one thing and got something they didn't expect or want. I understand the outrage there, because that was serious false advertizing.

Well besides ME3's endings, there's Final Fantasy XIV. SquareEnix released a broken MMO which went Free-To-Play weeks after release to appease some of the more loyal disgruntle fans. Then they decided just remake the game from the ground up, calling it FFXIV~Realm Reborn~, which fixes EVERYTHING that was wrong with the oringinal game.

The other thing would be Naruto Shippuuden--"The Invasion of Pain" arc (ch 403-449/ep152-175)

This might have been alot worse than the ME3 ending debacle, more or less EVERYBODY dies in this arc, which pissed off ALOT of Naruto fans. Besides the usual hate mail, the series creator Masashi Kishimoto (and few of his associates) got death threats, people threw stuff at them and spit on them whenever they were spotted in public, and I think there were few bomb threats. They were literally the most hated people in Japan at the time. That sh** was crazy.
A few weeks later their deaths were pretty much retconned, after Naruto defeated Pain, everybody was *magically* ressurected.

Lost (Season 2 to Season 3) Let's introduce a whole bunch of new characters, and kill them off before the mddile point of season 3. Along with most of the plots involving those characters.
Slider series ending.(Really, the whole last season was pretty bad..)
Quantum Leap series ending.(He just keeps leaping?!
Twin Peaks season 2
Mantis (season 1) After the pilot, they wanted to make it more mainstream. It was bad.
NYC Undercover(season 3) After three seasons, they wanted to make it more mainstream. It was even worse.
Enos(Really bad show)
Manimal(Even worse show)
But I don't recall any of those creators releasing documentary of the making of the series before laying such a bomb.

George Lucas went from probably one of the most beloved figures in all of Sci Fi fandom to being one of the most despised.

Probably the best analogy here. They created something great for two seasons, then systematically destroyed it. By the end it was a horrible mess that made no sense and satisfied nobody. I don't think ME 3's ending is quite as bad as BSG's was, but it's a similar sort of situation.
