Losing consumables and money on disconnects (either hosting player or losing connection to EA servers).
Losing connection to EA servers.
Load times resulting from disconnects.
Load times resulting from EA servers disappearing, taking you back to the main screen to then require you have to get back in to MP again = more load times.
Join a friends game loading to MP screen, then choose character then to the game to tell you its full, then dropping you back to MP screen so you can choose another option for load times.
Load times.
Game is excellent once you're in though.

And people who use med-gel when you are right next to them. Give me a chance to pick you up.

Not a problem with gameplay, but oh dear lord it takes forever to start playing after you click on the ME3 icon. First, the first stage of the game loads, before "press any button to continue". Then I have to wait for the EA network to connect, then I have to wait for the DLC's to verify. Both of course freeze the game to total unresponsiveness. When I finally get to click on the "multiplayer" option, and behold, I have to wait for the main MP menu to load. I choose my kit, click on the "quick game", and wait for the game to search for a lobby, then connect to a lobby and load a lobby. After that, another load for the map itself to finally appear.
Granted, I don't have the best computer with an SSD, but as far as the time between startup and gameplay is concerned, ME3 definitely takes the cake.
This to a huge degree. The initial freeze when the game boots up to verify DLC gets longer and longer with each DLC that comes out. And opening the store takes a pretty good chunk of time.
Beyond that, the fact that using objects/rezzing people is linked to the same button as running/jumping/cover. I wish we could have a button -- like E -- that lets us rez/take objectives'etc.
Would help so much.
This is quite possible to do on PC (and it's allowed by BioWare). Just search these forums and you should find several good guides. I've got mine set up for E to use, Left-Shift to run and dodge/cover on Space. Makes the game so much less annoying.

Personally it's when people screw up my biotic explosion set up and use either an arc grenade, or any tech ability. I don't want to use throw/shockwave twice, I just want my BE to detonate because if I have to do it twice, then my window to severly damage a group has closed. This is mainly evident when I use either the AA, Fury or Krogan Shaman. I personally don't mind a fellow adept detonating my explosions, I just usually don't like it when someone decides to throw a tech power on top of warp or reave.
Another is when it's gold, and no one wants to use consumables. Then when we actually get in game, those same individuals are missile and medi-gel horaders.
I don't have just one, I have several that are around the same level of making me angry or frustrated.
Just for my enlightenment, as a newish player, how can I tell what kind of power you are throwing? What are the clues I should be looking for? And how should I know if you have the ability to prime and detonate all by yourself? Just asking.

They might not realise you we're there. The downed camera doesn't really help either.

Sometimes I genuinely wish friendly fire was on so that, when I'm blazing away with a Revenant or Phaeston, and they'd wander into my line of fire so that they can shoot the enemies and stop me from doing it, I'd just gun them down so that they'd learn.
On a similar note, sometimes I'll be handling maybe a Hunter, a Rocket Trooper, and two regular Troopers by myself in a dug in position on the high ground in Giant where they stand no chance of killing me, only to have the entire team dead run over just to kill them, and leave the other two spawn points completely unmanned. Then I'm suddenly getting fire rained on me from Two Primes, Three Pyros, and Four Rocket Troopers while they run the other way wondering where the enemies are. I'm anchoring the right flank for a reason, just stay over there and you'll get most of the kills.

Pet abilities are too weak, have confusing evolutions, and there aren't enough of them.
That is to say, combat drones, sentry turrets, etc.
I want them to last longer, hit harder, have customization options available on the fly (attack weakest or strongest, healthiest or most damaged enemies first, stay close to player, stay far, stay in place [combat drone], etc. basically AI mode options).
I'd love a wider variety of them, too, with more interesting evolutions that have their mechanics plainly explained in the text. For example, the sentry turret has a base damage of something like 140 if you take the right evolutions, but is that 140 damage per bullet? 140 damage per burst of bullet fire? And how often exactly does it fire a burst, anyway? How often do rockets fire, and how often do flamethrowers fire, and for how long at a time?
We shouldn't have to trial-and-error this kind of stuff ourselves. This is basic information that should be listed with the rest.

- Geth stunnapalooza.
- All the loads and DLC authentications on start-up, AND on exit if I don't kill task. I usually start it up and spend 5 minutes doing something else, and come back, only to find I forgot to press a button on the title screen ARRRGGHH.
- The store takes about 15 seconds to load everytime... always been a bit but it seems longer than it was 2-3 months ago.
- Store trollin'.
- Joining a PUG lobby only to have the host leave instantly.
- Getting to a later wave and the host leaves... after migration, the other two players are gone as well.
- "Losing connection to the game" before I'm even connected to a game. Cue loading!
- Disconnects on Wave 10 (AS IF IT HAPPENS ANY OTHER TIME).
- Losing connection to the game because you lost connection to EA servers because ORIGIN HAD A DAMN HICCUP FOR 0.012 SECONDS. Cue connecting and authenticating and multiplayer loading!
- Charge / Shadow Strike not activating because the target is against the wall / on stairs / on a slight incline / hates you / is an Atlas.
- Really minor, and correct me if I'm wrong, but no character shouts "Praetorian!" or "Scion!" against Collectors like they do for other factions (i.e. Centurion! Marauders! GETH PRIIIIME!). I want it!
Still <3 ME3 MP.
EDIT: Oh, one pet peeve? lol TOO BAD

This + being left-handed. <_< (The controls are garbage on consoles.)
Other pet peeves: Geth, players playing for themselves (score/challenges) instead of teamwork, the sheer monotony of having only a single MP mode.

Salarian kill hogs are notorious for using Energy Drain on an enemy primed for a Biotic Explosion. The BE primed enemies are usually glowing blue-ish (although just because they are glowing doesn't mean they are still primed). The Energy Drain thrown on top of a BE primer basically masks the primer, so you have to get rid of the Energy Drain first before being able to detonate the BE.
As for knowing if a particular player has the ability to prime and detonate by him/her self, that just take experience.

Last night, after finally dropping a Hunter to end a silver wave, I felt compelled to unload five clips of graal spikes into it's corpse, because it was that frustrating to kill.
