I did all the upgrades(of course including the cannon) and was surprised to see him getting hit by a steel beam. Actually, he is the only one who dies throughout the whole mission! Does anyone have a clue how to stop this? It is really frustrating to see one of my favorite characters perish like that.

I'm pretty sure that the Ship Upgrades are the only things needed to prevent a death on the Normandy during the initial Omega-4 Relay push.
The deaths during the actual missions on the Collector Ship are based on your choices for the team leads and your squads loyalty.

That's what I heard. I just can't figure out why he keeps dying despite all the upgrades. It might be a glitch but not matter what I do it won't stop.

i assume you've done both ship shielding upgrades (from jacob(silaris) and tali(multicore))
if its always thane, take him to fight the occulus, both squaddies you take survive, if you lose someone else then you've got a glitch

Steel beam death is supposed to go with missing the cannon upgrade rather than armor or shields. OP, I'd double check that you really have got this upgrade installed by either talking to Garrus (he'll say he's done all he can if it's been done), checking your research console, or looking at the save Gibbed and seeing if the upgrade is checked.
