Undead Hamilote and Undead Turmoil both completed the weekend mission, and got no pack...still. Just posting this to see if it helps. Completed the OP around 1:30 EST. We both got the medal saying OP was completed.

Got my pack as well. As I don't normally do weekend ops (I believe this was just my 3rd or 4th), was there only supposed to be 3 items in it (N7 weapon, weapon mod, & ammo)? Not a complaint, just a Q.

Ok... i am confused...when i go to http://social.biowar...m/me3wchelp.php i get told that i was not awarded a Commendation pack for Operation Bloodlust.
Then i see the 3 Platforms of ME3.. (i only have PC/Origin) Gamertag is FuriousFurball and
Online Pass "yes".

I also see "Your account has not been granted a commendation pack for this operation." and "Create Review Ticket"

I click "Create Review Ticket" (there is no pop-up or anything) the page refreshes and it tells me that "Your report has been submitted. We will review your case within 48 hours and we will send our results to the associated e-mail address. "

and..

"Operation Bloodlust is over. If you have not yet received your commendation pack and believe you completed the weekend challenge please see the form below. Please note that this help page will only be available for the current operation until Friday October 26, 2012 at 10am PDT."

But i see no "form" below at all?

What should i do?

Still waiting on commendation pack - earned it last Friday evening...N7HQ confirms I earned it - but no pack as of now (751pm PDT)
