Hai Guyz,
My very first playthough of ME2, I had a renegade Male Shep do the sucide mission with no casualties without even trying. That was however before most of the DLC was released.
I have tried several times with my paragon femshep to dp the suicde run without losing any squadmates, However, after several attempts I always end up losing Jack(who I don't really care about, as her part in ME3 was very minor,unlike Garrus or Tali) who is not loyal. I could do her loyalty mission, but that would mean sacrificing half the crew.
So, my question is, is it possible to make an ending so that all the squadmates AND all of the crew survive(supposedly if you don't head to the relay immediately afterwards you lose either half or all of the crew)?
Is it possible to have a "Perfect" ending?

Yep, it can be done. Seems like your problem is Jack's loyalty but you haven't said set you don't have it. I'm assuming you did Jack's mission so did you side with Miranda when the two of them argued? If so we're you unable to resolve the argument with a paragon (I'm also assuming you were avoiding renegade options)? If the paragon response is greyed out then you need to get your paragon level up. Use an imported character, equip the Death Mask on the mission right before the argument, or max out your class passive skill and pick the evolution with the 100%morality bonus. Or all of them. With that lot in your favour you should be able to keep Jack and Miranda loyal. You can save the crew if you wake up Legion and talk to him until you get his loyalty mission as soon as you return from the Reader IFF mission. You should then be able to do the Geth station before the crew is kidnapped, so you'll then then able to rescue them right away.

Sounds like you missed out on the ideal moment to do Jack's loyalty mission and are now past the crew abduction point and don't want to go back to an earlier save.
You could try,
in the suicide mission, not assigning Jack any specific mission.
And for your later choices send Tali, Mordin or Kasumi (if you have her) as crew escort and use the other 2 as Shepard's teammates in the final battle (leave Jack with Grunt, Garrus and Zaeed holding the line and they should be able to protect her)

How exactly are you losing her? At which point during the mission?
You don't need all members to be loyal to have them all survive.
I just completed another playthrough without the loyalty of Legion or Grunt and everyone survived.

Hopefully, you've done all the ship upgrades.
If it's just Jack who's non-loyal, you can probably manage it. Don't put Jack in charge anywhere, send Mordin along to take the crew back in the end, and bring Kasumi and Tali to the final bossplace. That should stop anyone from dying when "holding the line".
Then you've just got to make all the right leader choices.
