If I remember correctly (totally possible that I'm off on some of the details), you need to go to Denerim. After you see Zevran introduced in a cutscene, wander around the world map and you'll get jumped eventually. Possible that there are some other requirements in there like needing to complete a major plot first; I've never looked up the Zevran scripting.

You need to do one of the 4 great story decisions, see the cutscene and get him at the first random fight you get.

When you say "do one of the 4 great story decisions" do you mean fully finish one of the major quests? For instance finish the Red Clif quest line then you will find him?
