As you know all there were 16 Mass Effect 3 Multiplayer Operations. And for each Operation
given one Commendation Pack - totally 16. I received all 16 Packs - 8 for Eagle
Pistol, 5 for Crusader Shotgun and 3 for Valiant Sniper Rifle. You can see this

- you can see that I have only 15 Gold Medals for Operations. I don't know why
I don't have Gold medal for the Operation Resurgence. All I know - I always played
ME3 Multiplayer in Bounty Weekends. So I want that in this page everyone can
see all 16 Gold Medals for Operations. Thank you.
