There are various theories about Flemeth's origins, in-game and otherwise. Some think she's an abomination, a possessed enchantress of ancient Fereldan. Others think she may even be one of the Old Gods, long since having taken human form, or perhaps the result of a ritual akin to Morrigan's.
However, looking at the evidence, I have a theory of my own. Seeing as how...
- She has very ancient and powerful magic abilities with a rather "forbidden" flair to them...
- She can shapeshift into a dragon, and how shapeshifters learn their forms from familiarity...
- Her grimoire has a "leafless tree", similar to that of the opening cinematic speaking of mages...
- She seems to have at least an awareness for the darkspawn and acknowledge their threat...
- And how she doesn't live far from Ostagar, the ancient fortress of the Tevinter Imperium...
I conclude that Flemeth may once have been a magister of ancient Tevinter, having survived the collapse of the Imperium and the fall of Ostagar, using mysterious arts she learned from her draconian deities to continue her existence through various hosts.
This does leave questions, however, as to how the Conobar story came about. Is it something that Flemeth herself cooked up as an "alibi", as it were? Or is a variation of the tale true, with "Flemeth" being the so-called "demon" that possessed the Flemeth of the tale, technically making her the first KNOWN "Daughter" of Flemeth, with the spirit of the Tevinter mage taking on her alias?
