Thanks Bryan but I do have one question. Is this challenge supposed to reset itself once you achieve it or is it a one time deal? I have completed the required 8 challenges and received the banner but none of the silver challenges done to aquire it have reset.

My squad Elite still says 4/8 even though I have 5 at 100% and Surivial Medals & W/End Challenge 1 do NOT say "number of times completed: 1."

Squad Elite NOT fixed.
It now says I've completed it, and I got the +1 to the mass effect banner, but now it says 8/8 and none of the medals reset.
This is Geth/Combat Mastery bug all over again :-(

It is fixed, not all the challenges are repeatable

if i understood the thing correctly .. all we had to do was to extract on any difficulty and it will count correct ? ... if that is the case .. i have actually extracted twice still not done

You also need to actually get a survivor medal

Got another medal, died and nothing changed.

EDIT: also extracted in one game with a medal obtained, still 6/8.

 this did not fix it  i still have 7/8 at 100% this is real fustrating  to say it is fixed but it is not

Tried for the third time a bronze duo, got the Gold medal, then died.
Still 6/8.

now my spectre mastery is bugged, 2/3 and all points are full.

also this is my 2nd completion, the 1st time it reset just fine.

edit add:
it apears that its the in-game medals earned that bugged this time since it only says 1 completion and the others say 2.

A firend of mine, Origin ID enris85 has the same issue: 100 medals, another one obtained, still 6/8!

Still broken for me.

Says 100/100 suvivor medals with 0 completions, although marked as complete but not counting overall.
I extracted twice tonight with survivor medals.
