Sounds like you forgot to copy the end004.bik into the "movies" folder of the main game.
BTW, if the installer doesn't work, most save way to go is to do the following:
Go to Origin, look at the game details for ME3, right click on the Exteded Cut DLC and choose "repair" in the context menu. After the repair is complete, do the same thing for the Leviathan DLC. After that, use the v0.3 installer to install MEHEM.

I couldn't remember whether or not I installed it before or after I eventually bought Leviathan anyway (I've not stomached getting to the end of the game since then so it doesn't really matter) so repairing was the safe route. If you repair each DLC then yes, it should be back to vanilla.
If you don't have your own back-ups, you'll need to repair 3 things: The Main Game, the Extended Cut, and Leviathan (if you have it). The other DLC are unaffected. Repairing the Main Game alone won't repair DLC, you must repair those individually from the addon/DLC page. Also note that Origin can't actually FIX the DLC files, it just redownloads the whole thing, so you're looking at 2-4 gb of downloading to perform the repair.

Had this idea after playing the Happy Ending Mod for Mass Effect 3.
Since they didn't have any scene for when the squad goes back to save
Shepard, I thought of making one.

Whoa, very nice. Where did that come from? I don't see it originally in this thread. I guess we could speculate that Cpt Fob is flying the shuttle. A problem might be all the custom Shepards, and maybe even distinguishing a male or female shepard in the game.Troxa wrote...
