Wouldnt it be neat if not only did your character make lasting impressions on the world and story, but it made lasting impressions on you character?
Bear with me:
Little jimmy is beset by eviiiil darkspawn, but alas the hero has lost his/her trusty favorite weapon and knows that to save little jimmy is to put him/herself in harms way. And so knowing that you most make a sacrifice of your character of some sort and choooose; you either save Jimmy getting banged up and mangled or dont save little jimmy and then you're an ass but an intact ass.
I think it would make for a great extra patina thing, also when talking about actions having consequences and coming with a cost. Wouldnt it make you pause before saving little jimmy if you knew your character might end up with a killer scar and a penalty to the strength modifier. This way the outcome effects your character very directly and you get to make a sacrifie. And hey, everytime you look in the mirrior and see that awesome scar you can be like 'ahh good old Jimmy...' or 'damn you Jimmy that was my favoritest arm...'
The effects could fade over time like in real life as to lessen the penalty or it could be like when your characters faint and come back with an injury. But if you knew that after saving jimmy you would have to fight a mean boss, it would make you consider again whether to take that blow, especially on hardcore mode.
Also instead of everything you do being a part of everything 'public' in the game this becomes your own thing, your own scar and tearfull story of how you saved little Jimmy. This gives you character some private life that is between you and it and maybe this story of painfull sacrifice is something that you only reluctantly divulge when your characters signifigant otter sees the scar for the first time.
Of course taken to excess it wouldnt be fun or invoke any kind of pathos or heroic feeling but just ruin the game and be invasive in you character design.
Thoughts? Or has all this been thought before and please forgive me if I am rambling.
