Well I think the proportion is slightly better than that, but it does seem that most of the time 2 out of the 3 people in a lobby are typically not gold-worthy while the 3rd person is decent or good, there are matches I find with more than just one other good player so that'd bring the average up a tiny bit.

Also, I exclusively play with PUGs since I don't have friends or care to make friends on XBL. PUGs are a true challenge, unlike the pansy option of playing with friends.

1 - when I am sync-killed and the rest of the team simply decide not to use medigel/rockets and is unable to finish the wave;
2 - when I'm playing with a very low level character (4-5) and I'm left alone (again, nobody using medigels) and have to solo a later wave (when I play with such a low level character, I'm pretty much able to handle myself, but not to solo);
3 - when I'm having really bad lag. Sometimes it is even impossible to fire a rocket before I'm killed again.

This week I'd been maintaining about a 90% clear rate in Gold PUGs, but last night I played four matches and all of them wiped between waves 6 and 10. Last one ended on wave 10 with my teammates running around the map like jackasses, ignoring the objective and getting insta-killed by Phantoms. Used up all my consumables trying and failing to do it myself. In lieu of throwing my controller through the screen, I turned off the PS3 and called it a night.

Collectors have the least success rate, followed by closely by the Geth, mainly on Goddess (I have more success with Geth condor and london surprisingly). If my team does looks a bit sketchy I'll just switch to my trusty Male Quarian Infiltrator. I only place the first kick if I suspect a rocket glitcher. I only leave a fresh lobby if the enemy is set to geth, or if someone has annoying background noise on their mic. N7, mic availability, and loadout doesn't matter to me for gold or lower. I've generally don't search public platinum, but I do expect high leveled characters.
Edit: I always put on my permanent gear before I search for a game. I usually put on full consumables, but not my best when I start the search. I'll adjust my loadout once I make into the lobby, but If I join a game in progress I feel I'll need some edge via buffs. The reason being since the team I joined had to three-man at least part of the game they could be low on equipment and would appreciate a strong new character joining their ranks. I will not put on my best stuff before the search though since I may join a match where everyone has already wiped, or join on a late wave where I won't get much use out of them.

Another 10% the team fails completely for some reason and I can't carry them (either because I am lagging, playing a dodgy loadout or just CBA)
That is a 30% flat fail rate.
Now out of the remaining games Id say 10% fails vs geth, cerberus and reapers and apprx 50% fails against Collectors.
Total fail rate would then be 30%+70%*(10%*75%+50%*25%) = 44%. Give or take a few ppm

I do examine everyone in the lobby. If I see low N7 I bounce asap. If I see a really bad loadout I'll bounce. If it looks like someone choose a bad weapon just for a challenge I bounce. If the Host does not ready up soon I'll bounce (nothing is worse than starting a game with an AFK host)
edit- I do have all my rares guns and gear maxed. I only mention this fact because that will skew the feedback you are receiving probably. Many of us with good manifest might have higher success rates. If I had to guess I'd say maybe 70-80% Success rate for me. If I get sync-killed some PUG I am on might fail asap. Factors like that

The one thing that tends to make them go wrong is mainly collectors. They slay pugs with amazing proficiency.
Other than that the biggest problems would be lack of knowledge about LZ resulting in partial extractions very often and geth bombers forcing players out of cover into stunlock, on open maps against geth.

if i am trying a new or very bad experimental setup it's like 1 in 4 times.
I always host when PUGing and mostly play against a set faction and unknown map, always on gold.
I also have no problem with finishing the wave alone.

I am a lot more picky now when selecting the party though... Generally want at least one other high N7/challenge point with solid weapons so i don't have to carry the pug.

PUG w/ my goto kits: Maybe 10% of the time. Probably less. I will say that if the team really sucks and I've solo-carried three waves, then dying on wave 8/9 feels like a relief. Unless I'm a krogan. My krogans will never give up. (Go to: QFI, HumEng, Krentinel, drellguard, et.al.)
PUG w/ other kits: One fail per seven games, I'd say. These are the kits never play or built weirdly. My first game with the geth soldier last night was like this. Damn I hate flamer. Luckily that wipe was on wave 10, three seconds after I finished the objective.

their consumables, and so long as a team does that it's usually pretty
doable to make it to extraction.
The biggest thing about most PUG games I've been in is not that they wipe all the time, but they do tend to be long. The average gold PUG is about 30-35 minutes, whereas 20-24 minutes is par the course for most of the non-random games I've played. It's usually because the average PUG team just doesn't seem to have as many high level rares or URs, may not use powers as effectively, and just doesn't kill enemies as quickly.
