Not exactly an epic fail.... but kinda botched a silver solo:

Started a silver lobby on my own, figured someone would join almost right away. Actually made it to wave 11 without anyone joining (kinda weird) but thought to myself

"Well at least I get my first silver solo!"

In extraction zone, 5..4...3...2..

Damn.

While trying to get into cover, I accidently locked my team in the reactor. Oops. 3 downs my fault.

Carry a device, thinking I'd be ok since I was on a tanky character, went down, mashed spacebar to not bleed out so fast, get ressed,
I didn't see the package on the ground, so I thought my resser had picked it up and delivered it already since we were just a few feet away from the drop off.
Took me a while before I realized I had instantly picked it up with my spacebar mashing. Those last few feet took at least half a minute because I was just standing there shooting things...

My most epic fail was shortly after the challenges first came out. I had just promoted all my characters to get to the first tier in the promotion rankings. I jumped on my QME and since he already had a weapon equipped and a name I thought I was good to go. Jumped into a Bronze match, headed to a spawn point and started spamming 3 for grenades, to no effect, I thought it was lag. It was not until wave 3 that I realized I never put points into grenades. Needless to say the Grenade Gear V did not serve me well that game.

I am always keeping an eye on the extraction time left, calling out 45, 30...and while doing this recently on Rio, completely forgot that I might wanna get back to the EZ...those enemy units can be quite distracting....

Dagger, walking backwards to EZ with pizza, you know, to move a little faster. Manage to get stuck on an object and downed by a brute. Luckily a teammate was there to pick up and deliver the pizza.

FB Reactor Hazard - managed to get fried in the core three times that session... people need mics and to call out when they plan to hit that there button.

Me as a shadow when Platinum was implemented.

Wave 1, I cloak, shadow strike the first phantom I see. She saw me, meleed, and followed up with a sync kill.

I was in a Reapers Gold match with my Paladin.  I believe it was wave 9.  It was towards the end of the round, and I was fighting a group of Ravagers, Brutes, and Marauders that were clustered together.  Killed them all to finish the wave, and started to walk away feeling like an action star in a Michael Bay movie.   I immediately collapse to the ground dead in between waves.  Killed by Ravager goo and swarmers.   

I cried inside.

Asari Valkyrie. Reapers. Ghost. Gold. I turn on AF and run through the spawn point on Wave 1. When I turn around I see nobody followed me set of the BE. I was first on the killfeed. By dying lol.

Hazard Giant, Gold, Collectors. Wave 9, I'm the only one standing trying to finish the wave. 2 Praetorians, Scion etc. I'm playing as a Krogan vanguard.
The last enemy is a Scion. He shoots cluster grenades, I try to move away only to get in cover, in the wrong side. The grenedes almost killed me, so I have to charge the Scion to replenish my barriers, plus, he was about to shoot with his cannon.
I charge, he makes his melee animation. I try to back up....NO, stuck against something..... He then proceeds to sync kill me.
It was the first time to be sync killed by a Scion. I did not get to see it, due to the agle of the camera!!
The shame.

Whenever I see my teammates being swarmed, pull out my missile launcher ready to be a hero, and get downed before I even get close to saving the day =(

That would be when I thought my Volus could be a ninja medic. Someone went down on Reapers/Goddess, I light melee'd to go invisible, and really thought I could run past that Banshee in the doorway to get to him.

Apparently, Banshees can see your soul straight through the invisibility cloak. Instead of being a hero, I got a hand through my chest and the team officially cut in half.

Felt like a real winner on that one.

for the past week or so i can't seem to hit the broad side of a barn with a cobra, i swear one time it even went through the legs of a prime.

Went to shadow strike an abomination and just as I triggered the strike he went apocalyptic; I had a few seconds to think "this isn't gonna be good."

To a squadmate:  "I'll save you" all heroic like, only to keep squatting on him because he died in cover and the one button kept getting me.

When playing different characters, subconsciously thinking my QFE muscle recall will make a magically toss a turret when all it does is biotic charge me into a Prime that's taken zippo on damage yet.

On one of my first gold runs, as last man standing, kiting a banshee for 5 full minutes while everyone watched offering helpful hints and I still never managed to take her out.  They took it as a mercy that I was finally dead.

Probably too many to list that I don't even remember.

yea, dozens of times.  It always happens when you need the grenades to keep from getting swarmed.    Might help if we could assign our three power keys for more consistent play.

I do this all the time!

Also, there was this one time on FB White, we were fighting Geth on Gold (I know, how original), and the entire team went down just after the wave 10 objectives. I stayed alive, and heard them saying on the mic something like 'Looks like he might finish the wave on his own', and other encouraging comments. Anyway, I saw a Geth Prime or two and thought: Missile!
Fired the missile...and missed.

Then theres all those times where I'm trying to shoot a Husk/Abomination with my Widow, and I keep missing. What a waste...

Shooting a missle, missing the preatorian (Bubbled at that!), The possesed Scion behind it, the Possesed Praetorians behind THAT, and missing the OTHR Possesed Scion behind THAT. And there was no straight line to go through all of them >.< (oh, and it was extraction time too, so EVERYONE saw me miss)
