The only real challenge is Grissom academy + some of the drawn out fights in London.
You'll be fine with whatever team you prefer. I personally go with Garrus and Javik much of the time, for amusing dialogues and out of personal preference.

If you were to remember one thing about ME3 on Insanity from this forum post. It'd be to practice your aim with the paladin because a certain marauder will show you no mercy.
I played through insanity without dying once when I first got the game using an Adept, Liara, and Javik for Biotic Combos... Up until I got to that last marauder, then I died about as many times as I died fighting the human reaper on insanity using a vanguard w/o any upgrades in ME2. :|

That marauder goes down pretty fast with headshots. The three husks before are arguably harder, cause, you know, three heads.
