My ending:
The Reapers finally conceded that getting rid of Shepard was the only way to overtake the galaxy, specifically humanity itself, and agreed that she would need to die believing she had won in order to make sure that she wouldn't fight to come back. Fortunately, with all the time Shep has spent around Reaper tech, the roots of indoctrination are already cemented in her mind. They were waiting to strike until she entered the beam, which is why the zapped everyone else who got close to it, making sure to leave her and her team alone as to avoid distracting her.
From the moment she enters the beam, they strike, bombarding her with images meant to cripple her mind further. The Illusive Man commits defies her will. Anderson dies. The Crucible doesn't fire. And then, the very image that has haunted her for the entire game appears as a friendly face, ready to assure her of her options. They are almost certain she will choose their "destroy" option, but provide her two others just to be sure.
But again they've underestimated her. She breaks free of their indoctrination and awakens in a pile of rubble where the beam once was. the teammates who were with her (in this case, Liara and Garrus) rush to her side, and help to pull the debris off of her. Anderson is not far behind.
Shepard explains that the beam was a trap, and tells Anderson she will take the Normandy up to the Citadel herself, provided that Anderson can cover her.
Insert a few "best damned pilot in the Alliance fleet" cutscenes, and you reach the Citadel.
After another long fight, you reach the Crucible control station through the citadel Tower, and provide the command, supplemental to Hackett's, to make it fire. The weapon cripples the Reapers, disrupting the signals of most of their tech. After hopping back on the Normandy you join the fight to conventionally destroy the Reapers once and for all. Shepard gives a speech, there are a few more badass cutscenes, and you succeed.
The epilogue, after the ending, is a cutscene that is very specific to your Shepard, as Bioware promised. If your lover survives the events above, you are rewarded. And the actions you've taken throughout each of the games reflect what is said about your Shepard in the final few scenes.
Best of all, I can tell you a good idea for the plot of ME4. Take Karpyshyn's idea about dark matter, and the struggle to stop the galaxy from eroding, and combine it with a side plot about Shepard disappearing after the war was won. Obviously, you'll be playing as another protangonist, but you can expect a few cameos, and interesting details to satisfy all the die hard fans. Hell, maybe Kolyat would finally end up as a teammate.
