-FB Ghost, I think it was with my SI, working on one of the achievements. Again, team is down, I am solo. Running for my life, killing the last remaining Marauders, a few Brutes and, yes, again chased by a couple of Banshees. I finally have it narrowed down to maybe one Brute and the final Banshee. She corners me in one of the rooms, the one with the hacking spot conveniently near an ammo box at the edge of the map. I turn around to run. My stupid SI instead tries to take cover facing the wall, thinking: "If I can't see her, she can't see me." ******. So he gets grabbed and game is over for all of us. I think I even still had one (1) missile.
Those are just two I can think off of the top of my head.
I've also had a lot of embarrassing moments not making extraction. Scoring the top spot, helping out left and right and generally causing mayhem, then completely forgetting to bug out. One match, against the collectors, on FB Dagger, I was on the other side of the map when I at least noticed the timer ticking down. I thought I could -just- make it, so I ran as fast as I could. Only there was one of those webby things in my way. I thought I could just run past, just to its side and maybe, just maybe, make it just in time.. I got stuck.

I was getting used to Silver still, back when this happened. Playing with friends, Uknown/Unknown turned out to be Reapers/Hydra. Wave 7 or 8, there was this one Ravager in particular down in the center of the map that was mercilessly dogging me and my friend. He'd go down, I'd revive him, get pounded by the Ravager, go down myself.
Wasted 2 ops packs and one medigel in that revive chain before I decided I'd had enough. I was an angry Turian Sentinel and I was going to go down there and punch that Ravager in the face. Repeatedly. Until it died. And I do mean punch - as in light melee only. I was THAT angry.
So, I did just that. Expended the rest of my medigel supply for stupid, petty revenge. It happened thusly: I started to approach the Ravager, he'd shoot me, I continue walking towards him like The Terminator, he shoots me again, I eventually go down about halfway to him. Rage-gel, continue my journey, get in the Ravager's face and pop his front sack. Start punching. Standing in Ravager goo. I eventually go down again. Use last, remaining medigel. Continue punching Ravager. He finally dies about one second before I do, at which point my friend revives me while laughing hysterically.
Felt totally justified in that, and still managed full extraction. At the same time, embarrassing that I let myself drop that much and then proceeded to burn through my medigel supply just because I had a grudge to settle.

It was kind of epic at the time, but embarrassing in hindsight. If that makes any sense.

In a Gold/Giant/Reaper PUG with three Krogans and a Shadow. I mention to the other krogans that it's safe to melee banshees as long as you're on a ramp. My exact words: "I read it on the forums." Wave 4, all krogans get sync killed on ramps. Shadow rage quits out of the match and the rest of us exit somewhere between the score screen and the lobby.
Would do it again.

It really is an effective trick. Much easier to succeed with non-krogans on maps like London and Vancouver though. You should go try it. No, really. :innocent:

Also, another one that wasn't me but a friend, he was the last one left on Goddess and extracting. Collectors were everywhere. He got picked up by the Praetorian a couple time and the animation finished each time but he kept just going down so he would medi gel and try to jet pack away, but get grabbed again. And finally died when he got away after using all his gel and got collector pickaxed to death. So whe have surmised that the Havoc has the hardest Helmet in the galaxy.

The sad part is I do eventually want to unlock other banners... which means I gotta eventually started doing 200 waves with the other 99% of my manifest... but it's so hard. I feel like I'm cheating on my Turians.

Being elbowed between 3 Marauders... they chain stunned me till I went down and then proceeded to stomp me out... I will never forget... -.-
Okay, that actually did remind me of a more recent embarrassing death... which, of course, totally had to happen on that one magical late afternoon that Derek Hollan graced my PS3 lobby with his presence. It was he, my buddy Rad Rage, and I... and one extremely lucky PUG... playing our last Silver game of the day.
I chose the Turian Havoc because Derek actually named that kit, so I did that as a sort of "salute" to him (yes, previous rounds were with my Turian Sentinel, go figure). My friend was the Batarian Vanguard, Derek was the Destroyer, and I don't remember what the PUG ran. We did Giant/Geth/Silver and I got a little too Havoc Strike-happy. I ended up in a group of hunters and troopers at the LZ area and proceeded to get staggered... and battered back and forth like someone getting beat up in a circle of schoolyard bullies. Which is exactly how I described it over the mic. And then I went down, whereupon I immediately rage-gelled... then felt bad because Derek's Destroyer was running up to be the hero (I hear this little "Awwww... I was gonna get you back up" over the mic as my Havoc stands back up, LOL). I then thanked him for his efforts anyway, trying to come save the "damsel in distress". Got a good laugh at that.
