Scariest? It would have to be my first gold match ever, turning a corner to escape an atlas's rocket just to walk right into a spawning party of 4 phantoms.

I basically tried to scream, say "oh sh_t" and cry at the same time.
Turns out you can't do that, I choked on my tongue, blubbered like a newborn baby and fell out of my chair in astonishment while watching my qfe being sliced into ribbons and palm blasted to death.

Was about to say "Reaper Gold" 'cause the Banshee screams sound in my ears for minutes after a match. I have two Reaper Gold and feel I need a break. It's more unnerving.

But silent Banshees are really scary. I got one in a solo match this weekend - weird. I ran circles on Giant and aimed at some Cannibals when something grabbed me:

That moment, when in the middle of round 8, 3 of your pug are dead, you're hiding from a banshee and some nasties while reloading, and as you're about to pop out of cover to check the situation, one of the pug members finally decides to chat, and yell's "GO DUDE!!" in what can only be described as the creepiest, most trouser soilingly startling voice ever..

When my weapons and powers stop firing - when they are fully loaded. Happens to me once in while. When this happens, the only thing I can do is melee/heavy melee and walk - cant even run.

Usually the problem is gone when the next wave starts or after carrying a pizza

Banshees screams disturbed me until i fully achieved the silver->gold transition. I was scared, didn't know what to do, their cycles, and they kept screaming. Needless to say, i was shaky when my teammates ended up dead before me on a reaper wave!
Thankfully i learnt how to manage that by seeing some good players kite them around while i was dead.

Nowadays i tend to think the scariest thing is the dragoon+phantom combo. That or a UUG pug facing a gold Collectors' match.

Gang rape! I SSed once into a reaper spawn full of Brutes, elbows and horny husks. Not as glorious as the Dragoon/Phantom party but still feels similar.
