I remember reading it somewhere before ME3 came out that there's supposed to be one where you have to choose between your LI and Liara. It was probably on Mars. I can't remember where, and the thread it probably gone now. I think it was cut after the leak.

It was suppose to take place on Thessia - it was from a much older version of the script (back when the VS had a partner and was still convinced you were a Cerberus traitor even after the Citadel Coup and Javik was the Catalyst), where Shepard was injured and had to crawl to either the VS or Liara to save only one of them while the temple was collapsing.

That would have been fun.  Well, not for Ashley though... ha!

Yeah... should've been more forced deaths in ME2/ME3. But BW tried to please the Happy Ending Brigade.

Yeah, pretty much. Honestly, I don't necessarily like how they were portrayed, but I was happy that the actual endings to the series were so morally grey, as opposed to, "Make this terrible choice! ...unless you have a super-high Paragon/Renegade score, because then you can do whatever you want! YAY"

Mass Effect 2 got hit a little too hard by happy endings. It didn't really fit the plot, or the series, heck, it was called "The Suicide Mission" even. A flawless victory shouldn't have been possible, it's not fun winning like that, and even the tiniest bit immersion-breaking.

I think this is fair regarding ME2, the suicide mission really didn't do it's name justice. But we do have a bunch of people dying in ME3 and few would call the ending happy, so I don't think it's really fair here.
