Despite the patch stating the following fix:
"Leliana's personal plot can now be completed if the player
substantially increased her approval (through gifts) before talking to
her."

I still have no dialog option about spies or anything. I have 100 approval and we're "in love". About the only thing I can do is tell her to leave.

So if the patch didn't fix this, what will? Is there any way to patch it myself via a mod?

There's an encounter you have when your travelling between locations with her in your party that triggers her quest. It worked in the game I finished as well as my current one (1.02a patched). Presumably it happens once you've increased her approval rating enough that she tells you the full story about why she joined the Chantry.

You need to have Leliana with you when you travel, this is the way it is triggered.

I don't have the random encounter because I can't ask her about the spies. She has always been a member of my party since I've met her...she is my lockpicker and trapdisarmer.

Here is a blurb from the wiki:

Talk to Leliana after Lothering once her approval is around 30+ and make sure to ask her about "minstrels and spies in Orlais." This should prompt a random encounter that starts her quest after you leave camp with her in your party.

Her dialogue options regarding Marjolaine may not appear if her approval is too high and/or you are in a romance with Leliana. In this case, save and try breaking up with her to see if you get the dialogue. Updating the game to patch 1.02 should also fix this bug.

When above mentioned solutions to get Leliana's quest do not work, there is option to use console command to lower Leliana's approval (and rise it after getting the quest). Namely, without quotes, "runscript zz_addapproval 9 -50" lowers Leliana's ("9") approval by 50. To get approval back: "runscript zz_addapproval 9 50". Also, it has to be noted that there is console command "runscript zz_app_debug" that starts approval debug helper where you can lower/raise approval or check some options (like romance) - this command should be used with utmost care and as a last resort as it could mess up entire game.

Will running the console command flag me as cheating and ruin achievements? I don't know how to break up with her otherwise.

To break you with her, all you need to do is ask her about something personal, and then tell you need to end things.  If you don't have that option, you aren't in a romance with her, and would need to use the console commands.  You won't get flagged as a cheater or anything, but you do need to be careful you don't mess something else up.

BTW, did you start your game pre or post patch? Its possible that the patch just makes it so that things work from scratch, but can't go back and fix people who are already far enough along.  (Or possibly this part of the patch just didn't work)

I started playing pre-patch, so yes, maybe that's why it's still bugged for me. I will attempt those steps, thank you.

If you break up with her, to get the quest going, can u still become romantically involved with her again?

I'm playing on PS3. I ran into the same problem: Was romancing her and not getting the option to ask about the bards. So, I broke up with her. But, while doing so, there are options to choose that say you are doing it because you're worried about losing her. Not sure if this makes a difference or not, but, it ended with them saying to each other that they can pick up the romance later. Afterwards, her approval dropped -25, but, right after I asked her about the bards and once she finished telling me everything, her approval went up +3. So, I figure there is a good chance of starting the romance again.

the fourth patch has been released and bioware still hasn't fixed this. ****ing idiots. bioware has a talent for bugs which really annoy the archdemon out of me. just like in mass effect, now in da:o i am pissed off about their incompetence. the game is awesome, but it seems to me that bioware sucks at patching their games.
